<p style="text-align: justify;"><strong>Bengaluru </strong>: <strong>Dr B H Banarji,</strong> Senior Consultant, Department of Orthopedics of Sakra World Hospital has created a milestone in arthroscopy by inventing a new form of effective, simple and durable method of knotting, known as the<strong> Banarji Knot.</strong> This knotting method is a low profile, non-bulky, more secure, double slide locking knot which can be utilized in arthroscopic surgeries. It has been used in more than 250 surgeries till date and has been found to be very effective. <strong>The analytical research study on the Banarji&#8217;s Knot is published in the Journal of Arthroscopic Surgery and Sports Medicine in October, 2021.</strong></p>
<p style="text-align: justify;"><strong>Banarji Knot, an idea initiated to benefit the patient’s surgical outcome, is specifically used for arthroscopic surgeries – knee or shoulder repair, where tendon repair or ligament repair is needed.</strong> This knot was aggressively tested by a bio medical testing lab at the Indian Institute of Science, Bangalore and approved by the authorities for surgical procedures and proves to be better than other existing prevalent knots.</p>
<p style="text-align: justify;">Arthroscopic knot tying is a crucial component for a successful arthroscopic surgery. For an effective knot in the arthroscopic procedures, it is important for a knot to possess the following characteristics such as, ease of application, reproducibility, ability to slide through arthroscopic cannulas, ease of setting the lock, knot profile, and reliable initial security. <p style="text-align: justify;"><strong>Bengaluru, October 29, 2021:</strong> Observing World Stroke Day on October 29, Sakra World Hospital, one of the leading healthcare providers in Bengaluru and India’s first FDI hospital, organised a walkathon to raise awareness on stroke and a free stroke check-up camp for its employees and the public on October 29 and 30, 2021. Sakra World hospital is also geared up to flag off its <strong>Comprehensive Stroke Clinic on October 30</strong> at 8:30 AM that would offer complete stroke care services with an aim to effectively diagnose, treat, manage and prevent stroke and other stroke related severities in the presence of Dr Pratima Murthy, Director of National Institute Of Mental Health And Neuro Sciences (NIMHANS), Bengaluru.</p>

<p style="text-align: justify;">The walkathon that begun at the wee hours of morning at 7:00 AM from Sakra World Hospital towards Kadubeesanahalli Bridge and back to the hospital had 150 participating employees followed by screening of the employees and others for stroke risk factors like cardiac problems, hypertension, diabetes and lipid disorders at the free stroke check-up camp in the hospital.</p>
<p style="text-align: justify;">Elaborating on the need to raise awareness about stroke,<strong> Dr Shiva Kumar. R, Senior Consultant Neurologist &amp; Epileptologist said,</strong>“We organised the walkathon to raise awareness about the need to identify symptoms of stroke, act quickly and arrange medical intervention without any delay. This would help people act faster and save their loved ones from death or a possible disability. On the other hand, at the stroke camp, our focus was on personal habits which increase the risk of stroke like smoking, alcohol and drug abuse. We had a calculator to assess the risk of stroke in an individual for risk stratification. The camp also covered stroke screening health packages like Carotid Doppler, MRI Brain with Angiogram, CT Angiogram, Specific blood tests for identifying the mechanism of stroke and the root cause. On prior appointments, we offered free neurology consultations and discount on stroke screening packages at the two-days-long camp.”</p>
<p style="text-align: justify;">Both the walkathon and the free check-up camp were conducted by strictly adhering to the Covid-19 protocols like prior appointments, compulsory wearing of masks and appropriate seating arrangements and distance maintenance.</p>
<p style="text-align: justify;"><strong>Stroke, as we all know, is a very common problem and can be treated and cured efficiently in the early stages.</strong> In case of stroke, the time plays a crucial role as the first 4.5 hours following an ischemic stroke is the golden period for administering ‘clot buster’ medication which is life- saving. Beyond the first 4.5 hours, treating options become limited and mechanical clot removal (thrombectomy) can be tried for up to 24 hours in selected cases. Stroke can affect anyone across all age groups and can be almost completely recovered with effective medications and rehabilitation.</p>

The seizures were characterized by abnormal behavior lasting for a minute followed by jerky movements involving the right hand along with a brief period of loss of consciousness also lasting for a few minutes. These episodes were very few initially, but for the past 2 years, she started getting a minimum of 3 to 4 seizures per day in spite of 3 antiepileptic medications.  Due to the recurrent seizures and chronic medications, her quality of life was deteriorating day by day.</p>
<p style="text-align: justify;">At Sakra World Hospital, Lakshmi consulted <strong>Dr Satish Rudrappa</strong><strong>, Director – Neurosciences and </strong><strong>Dr Shiva Kumar R</strong><strong> – Epileptologist</strong>.  Upon investigation, she was diagnosed with right <strong>Mesial Temporal Sclerosis (MTS)</strong> and a small Right Frontal benign <strong>brain tumor. </strong> To decipher the exact cause of her recurrent seizure, series of Video EEG, MRI and PET scans were taken to localize the origin of seizures as the right temporal lobe<strong> (Temporal Lobe Epilepsy).</strong></p>

<p style="text-align: justify;"><span style="color: #ff0000;"><strong>What is Mesial Temporal Sclerosis (MTS) ?</strong></span></p>
<p style="text-align: justify;">MTS is sclerosis of the deepest portion of the temporal lobe similar to the scarring of any tissue in the body which gives rise to seizures. It requires experience and expertise to identify such types of unique seizures called <strong>Temporal Lobe Epilepsy.</strong> In Lakshmi’s case otherwise, the illness would have been easily attributed to a small meningioma growing in her brain which is very benign and not the cause for the seizures.</p>
<p style="text-align: justify;">“At Sakra, we receive more than <strong>600 epilepsy patients every year.</strong> Out of which 150 of them are termed as “refractory epilepsy”.  About 50 patients of these refractory Epilepsies’ turned out to be surgically remediable epilepsies.  In Lakshmi&#8217;s case, the development of MTS could be due to the early seizures she suffered during her childhood which were not addressed or monitored during the early phase leading to scarring,” said <strong>Epilepsy specialist Dr Shiva Kumar.</strong></p>

Various methods have been hitherto available to deal with this situation, such as rotablation , very high pressure balloons , atherectomy devices etc. A very recent addition to the armamentarium is Shockwave lithotripsy also called <strong>intravascular lithotripsy</strong>, which uses a special balloon to deliver high pressure waves at the blockage and breaks the calcium. This softens the blockage and facilitates successful angioplasty and stenting.</p>

Dr Chandrashekar P, Sr. Consultant &amp; HOD – Orthopaedics at <a href="https://www.sakraworldhospital.com/" target="_blank" rel="noopener noreferrer">Sakra World Hospital</a>, Bengaluru has </strong>created a milestone in knee surgery along with his team of doctors by devising a new technique to create a portal/passage. This surgery has been developed to approach the upper portion of the outside ligaments of the knee using keyhole surgery technique. Clinically referred to as <strong>Direct Lateral Portal</strong> and also known as <strong>“Bengaluru Chandrashekhar Portal”</strong>, it is an extension of an arthroscopic surgery developed for the first time in the world to address the health problems occurring in the Outside or Lateral Collateral Ligament (LCL) of the knee. LCL are the two surrounding connective tissues that connect the knee bones together, keeping it stable.</p>
<p style="text-align: justify;"><strong>This newly devised portal serves multiple purposes &#8211; it can be used to address the pathologies of all types of injuries or broken LCL, Avulsion Fractures, Calcium Apatite Deposition <a href="https://healthvision.in/category/articles/diseases/" target="_blank" rel="noopener noreferrer">Disease</a> (CADD), Popliteus and Anterolateral Ligament (ALL) Reconstruction.</strong></p>
<p style="text-align: justify;"><img decoding="async" class="alignleft wp-image-4707 size-medium" src="https://healthvision.in/wp-content/uploads/2019/12/Photo-2-200x300.jpg" alt="r Chandrashekhar P and his team at Sakra World Hospital" width="200" height="300" />Despite medical and surgical advances in the field of knee surgery, the normal practice has been through open surgery. No defined portal or entry technique was developed till date to reach the area in the LCL of the knee. There is a need to create a passage to address the different pathologies. But, narrow ligaments and multiple blood vessels in the knee pose a significant challenge for surgeons to operate. Using keyhole surgery continues to be difficult while trying to reach the upper part of the pathology and calcific deposit in the LCL. After years of research<strong>, Dr Chandrashekhar P and his team at Sakra World Hospital discovered a new passage to reach the point in the LCL without any struggle using keyhole surgery technique.</strong></p>
<p style="text-align: justify;">This technique was conceptualized to prevent tearing of the ligaments during open surgery and was conducted on a cadaver study on both the knees at a cadaver lab and further dissected to know the safety of the portal. The process involves incision to reach the part in the outer ligaments of the knee without damaging the LCL fibres and peroneal nerve (a branch of sciatic nerve that originates from the lower spine required for movement) while making the portal. Cases of pathologies in the LCL are rare and occur in about 1 in 1,000 cases.</p>
<p style="text-align: justify;"><img decoding="async" class="alignright wp-image-4708 size-medium" src="https://healthvision.in/wp-content/uploads/2019/12/Bengaluru-Chandrashekhar-Portal-300x218.jpg" alt="Keyhole Passage for Outside Ligament Pathologies of the Knee" width="300" height="218" />Speaking about his discovery of the technique, <strong>Dr Chandrashekar P., Sr. Consultant &amp; HOD – Orthopaedics, Sakra World Hospital</strong> says, “Almost 70% of the knee and joint surgeries are conducted through keyhole surgery since they are less painful, involves less incision and scar, ensures speedy recovery and reduced hospital stay. For the past three years, we have been constantly researching to find out a new portal to address the pathologies in the LCL. Without the knowledge about the portals, it becomes difficult to reach to this particular area of the knee and joint. With this new technique, the aim is to create an entry to the LCL region of the knee without any hassle. This new discovery is dedicated to the people of Karnataka and has been named as ‘<strong>Direct Lateral Portal/Bengaluru Chandrashekhar Poral’</strong>.”</p>
<p style="text-align: justify;">“My health condition had started to worsen over the time despite undergoing various treatments. My movement had become restricted, due to which I become highly dependent on my family members to carry out my daily routines. I met Dr Chandrashekhar at Sakra World Hospital who explained the health condition I was suffering from in a simpler way and cleared my doubts about the new type of surgery that he suggested to conduct. Not only the surgery improved my health significantly, it helped me regain my strength to move without any support. I would like to thank Dr Chandrashekhar for giving me a new lease of life,” said, the patient.</p>
<p style="text-align: justify;">The new technique has been accepted and published in the <a href="https://medicalresearchjournal.org/index.php/GJMR/article/view/1851">Global Journal of Medical Research</a> that serves as a knowledge-sharing purpose for the medical fraternities globally.</p>
<p style="text-align: justify;"><strong>Case Scenarios:</strong></p>
<p style="text-align: justify;"><strong>Case 1:</strong> A 50-year-old lady presented with pain in the left knee of one-year duration. There was no improvement with conservative treatment in the form of anti-inflammatory medication and physiotherapy. X-ray of the knee showed calcification in the LCL region. MRI scan showed intra-substance calcification in the LCL near the femoral attachment and associated medial meniscus root tear. With no improvements with medications and conservative methods of treatment for one year, the patient opted for our treatment plan of arthroscopic meniscal root repair with concomitant arthroscopic excision of LCL calcification.</p>
<p style="text-align: justify;"><strong>Case 2:</strong>  A 54-year-old lady presented with left knee pain of 6 months duration. She was treated with anti-inflammatory medications and physiotherapy. But the symptoms showed no improvement. X-ray showed calcific deposits in the lateral side of the knee. MRI showed calcific deposits at the femoral attachment of LCL. Since her pain interfered with daily activities, arthroscopic excision of calcific deposits was considered.</p>
<p style="text-align: justify;">Both cases were operated after institutional ethical committee clearance. The two cases reported here were operated using the new &#8220;Direct Lateral Portal/Bengaluru Chandrashekar Portal&#8221; after obtaining written informed consent with a prior explanation of the procedure<strong>. Dr Chandrashekar P is one of the most revered doctor of the country and is a mentor for many across the globe. He is the first Orthopedic surgeon to start knee resurfacing in South India and has completed over 10,000 knee surgeries.</strong> Also, he is the first Orthopedic surgeon to start Minimally Invasive Computer Assisted Knee Replacement Surgery in Karnataka.</p>
