Often seen as quick and effective solutions, they are easily available and frequently used without medical supervision. However, ophthalmologists across India are increasingly warning that prolonged or unsupervised steroid use is emerging as a major trigger for secondary glaucoma, a sight-threatening condition that can cause irreversible blindness.<br />
Steroids—whether taken as tablets, inhalers, skin creams, or eye drops—can raise intraocular pressure when used for extended periods. What makes this especially dangerous is that the rise in eye pressure usually occurs without early symptoms. Patients often feel normal while the optic nerve sustains permanent damage.</p>
<p style="text-align: justify;"><span style="color: #0000ff;"><strong>India’s Growing Glaucoma Burden</strong></span><br />
India already bears a significant share of the global glaucoma burden. An estimated 12–13 million people in the country are living with glaucoma, accounting for nearly one-sixth of cases worldwide. Globally, glaucoma affects around 75–80 million people, with projections suggesting the number will exceed 110 million by 2040.<br />
Despite being the leading cause of irreversible blindness, glaucoma remains widely underdiagnosed in India. Experts estimate that 85–90% of cases go undetected, largely because the disease progresses silently. By the time patients notice vision loss, irreversible optic nerve damage has often already occurred.</p>
<p style="text-align: justify;"><span style="color: #0000ff;"><strong>Steroid-Induced Glaucoma: A Preventable Threat</strong></span><br />
In recent years, doctors have reported a steady rise in newly diagnosed glaucoma cases. This increase is driven by longer life expectancy, higher rates of diabetes and myopia, improved diagnostic tools, and greater public awareness. However, a growing proportion of these cases are now linked to secondary glaucoma caused by steroid misuse—an avoidable risk factor.<br />
Steroid-induced glaucoma is increasingly seen in routine clinical practice, particularly among patients who have used steroid eye drops or medications for long periods without supervision. Because the condition develops silently, patients are often unaware that treatment for one problem is steadily damaging their vision.</p>
<p style="text-align: justify;"><span style="color: #0000ff;"><strong>Who Is Most at Risk?</strong></span><br />
Glaucoma is most commonly diagnosed in individuals over 40 years of age, with incidence peaking between 50 and 70 years. However, ophthalmologists are now detecting juvenile and early-onset glaucoma, especially among people with a family history of the disease or secondary triggers such as steroid exposure.<br />
While primary open-angle glaucoma remains the most common form, steroid-induced glaucoma and pseudoexfoliation glaucoma are being reported with increasing frequency. Individuals with diabetes, hypertension, thyroid disorders, high myopia, long-term steroid use, or a history of eye injury—even from childhood—face a higher risk and require closer monitoring.</p>

Adding to the problem are common myths—that good vision equals healthy eyes, that glaucoma affects only older adults, or that normal eye pressure rules out the disease. In reality, glaucoma can develop at any age, and eye pressure alone does not tell the full story.</p>

<p style="text-align: justify;">Glaucoma is the name given to a group of conditions in which the optic nerve sustains damage where it leaves the eye. In many cases, this damage is associated with a rise in pressure within the eye. As the pressure builds, it compresses the fragile blood vessels supplying the optic nerve, which sends messages to the brain. <strong>If left untreated, this can lead to irreversible damage to the optic nerve.</strong></p>
<p style="text-align: justify;"><span style="color: #ff0000;"><strong>What Causes Glaucoma?</strong></span></p>
<p style="text-align: justify;">A clear liquid called aqueous humor flows through the eye nourishing the lens, iris, and cornea. When the delicate mesh that forms the drainage system becomes defective (called open angle Glaucoma) or completely blocked (called closed angle Glaucoma) the liquid continues to flow into the eye and pressure builds. It is this build-up of pressure which causes Glaucoma.</p>
<p style="text-align: justify;"><span style="color: #ff0000;"><strong>Recognizing the Symptoms </strong></span></p>
<p style="text-align: justify;">The most common form of Glaucoma, open angle, starts gradually with darkening at the outer edges of your sight (peripheral vision) and gives no warning symptoms until a very late stage, when much irreversible damage has been done to the field of vision.</p>
<p style="text-align: justify;"><span style="color: #ff0000;"><strong>Without treatment it can progress to complete blindness</strong>.</span></p>
<p style="text-align: justify;">A much less common form of Glaucoma, acute closed angle, starts with severe pain in the eye, headache, blurring of vision, halos around sources of light. <strong>This needs immediate treatment</strong>.</p>
<p style="text-align: justify;"><span style="color: #ff0000;"><strong>Who is at risk ?</strong></span></p>
<p style="text-align: justify;">It is important for each of us, from infants to senior citizens, to have our eyes checked regularly, because <strong>early detection and treatment of glaucoma are the only ways to prevent vision impairment and blindness.</strong> This may apply to you if: You are 55 years &amp; above and have not had eyes examined regularly Someone in your family has a history of glaucoma Myopia (nearsighted)</p>
<p style="text-align: justify;"><span style="color: #ff0000;"><strong>Diagnosing </strong></span></p>
<p style="text-align: justify;">1. An annual eye examination is as important as a regular vision test. This examination will measure both the pressure within the eye, fundoscopy (Cup/Disc ratio) and the field of vision.</p>
<p style="text-align: justify;">2. These tests together are more effective in the detection of Glaucoma than the standard eye test alone.</p>
<p style="text-align: justify;">3. A regular test of the pressure within the eye can detect Glaucoma. (Tonometry)</p>

<p style="text-align: justify;">Although treatment can control Glaucoma, and possibly avoid or delay the need for surgery, sight loss cannot be recovered so early detection and treatment are vital.</p>
<p style="text-align: justify;">Most drug treatments aim to reduce the production or drainage of fluids in the eye. It is very important to use prescribed medication regularly and not to miss doses.</p>
<p style="text-align: justify;"><strong>1. Laser Treatment: </strong>If the condition cannot be controlled using medication, it may be necessary to have laser surgery. The drainage holes are opened using a laser beam and the pressure within the eye is then controlled with the appropriate medication.</p>
<p style="text-align: justify;"><strong>2. Surgery: </strong>An eye specialist may recommend surgery to help control Glaucoma through which the aqueous fluid can escape from within the eye and drain away under the loose covering of the eye (conjunctiva).</p>
<p style="text-align: justify;"><strong>3. Eye Care: </strong>Eye drops should be applied correctly to be of maximum benefit. Consult your doctor for correct administration of drops.</p>

<p style="text-align: justify;">It is not curable, but it is manageable usually with simple medications such as eye drops and pills.</p>

<p style="text-align: justify;">All age groups are at risk , though elderly persons above the age of 60 years are more at risk.</p>

<p style="text-align: justify;">Surgery is done when medications are not effective or they cause intolerable adverse effects. Surgery is effective in relieving , but the condition can recur even after surgery.</p>

It should be diagnosed and treated on time as the progression could cause vision loss.</p>

<p style="text-align: justify;"><strong>Type 2</strong> – As soon as the diagnosis is done, the yearly examination should be started</p>
<p style="text-align: justify;"><strong>Pregnancy </strong>– Pregnant women with Type 1 and Type 2 diabetes, should carry out eye examination before planning for pregnancy or in the first 3 months.</p>
<p style="text-align: justify;"><span style="color: #ff0000;"><strong>Treatment for eyes </strong></span></p>
<p style="text-align: justify;">The treatment depends on the type of eye disease you are diagnosed with.</p>
<p style="text-align: justify;"><span style="color: #ff0000;"><strong>1. Eye injection with medication</strong> &#8211;</span> To prevent the abnormal growth of blood vessels anti-VEGF medication is injected into the eye. It has medicine like Bevacizumab , Ranibizumab . The eyes are numb to prevent pain and the medicine is injected with a very fine needle into the eye. <strong>This treatment prevents fluid leakage and also improves eye vision.</strong></p>
<p style="text-align: justify;"><span style="color: #ff0000;"><strong>2. Cataract surgery</strong> –</span> Surgery is indicated in patients who have developed a cataract. The cataract is removed by replacing the cloudy lens with an artificial lens thus improving vision.</p>
<p style="text-align: justify;"><span style="color: #ff0000;"><strong>3. Laser treatment </strong>–</span> To prevent the progression of eye disease, and vision loss laser surgeries are recommended. However, the eye injections (anti-VEGF) treatment approach is better to bring back vision loss. Laser treatment could be done for Glaucoma.</p>
<p style="text-align: justify;"><span style="color: #ff0000;"><strong>Diabetes treatment </strong></span></p>
<p style="text-align: justify;"><strong>1. Once you are diagnosed with diabetes</strong>, the chances of development of eye problems increase. It is important to get a regular eye examination done and to maintain a check on blood sugar levels.</p>
<p style="text-align: justify;"><strong>2. Control your blood sugar levels</strong> by maintaining a healthy weight, taking your medications on time, refraining from smoking, and maintaining good cholesterol levels with a healthy diet and regular exercise.</p>

<p style="text-align: justify;">High blood glucose levels can cause changes in the shape of the lens which can temporarily cause blurring of your vision. This commonly occurs before being diagnosed with diabetes or when diabetes is out of control. When blood glucose levels are reduced through appropriate treatment, the blurriness usually disappears. <p style="text-align: justify;">India can easily be the world’s capital of glaucoma, a group of eye disorders that leads to irreversible blindness, with the disease affecting 12 million people, and remaining undiagnosed in over 90% of the cases.<strong> Increasing population of senior citizens, diabetics and people with refractive errors, the known potential risk factors</strong>, are set to make glaucoma a rapidly emerging non-communicable ocular disease in the country, which already houses 20% of the world’s blind.</p>
<p style="text-align: justify;"><strong><span style="color: #ff0000;">No known measures to prevent glaucoma</span></strong></p>
<p style="text-align: justify;">There are no known measures to prevent glaucoma. The only way to prevent blindness from glaucoma, which shows no symptoms in the early stage, is early diagnosis and treatment that could halt the progression of this <strong>silent thief of sight. </strong>Nearly <strong>40 million people in India, including 1.6 million children, are blind or visually impaired.</strong> Next to cataracts, and untreated refractive errors, glaucoma is the third leading cause of blindness in India. However, glaucoma is more complex and dangerous as the sight loss caused by it is progressive and, unlike cataract, is irreversible.</p>
<p style="text-align: justify;"><span style="color: #ff0000;"><strong>Glaucoma is both under-diagnosed and under-treated</strong> </span></p>
<p style="text-align: justify;"><strong>Glaucoma</strong> is both under-diagnosed and under-treated. It occurs when the pressure inside the eye (Intraocular pressure) increases. The pressure is normal so long as the rate of production of aqueous humor, the clear liquid inside the front part of eyes, matches the rate of its drainage. However, when the channels carrying this fluid get blocked, the pressure inside the eye increases. This increased pressure damages the optic nerve, which passes the visual messages to the brain.</p>

<p style="text-align: justify;">There are two major types of glaucoma:<strong> open angle glaucoma, and close angle glaucoma.</strong> Open angle glaucoma is the most common form of glaucoma. It accounts for at least 90% of all glaucoma cases. It progresses very slowly and the patient may not feel any loss of vision until it affects the central vision. This results in permanent damage if the patient does not get an eye check-up.</p>

It falls on 8 th October 2020. It is coordinated by <strong>International Agency for prevention of blindness (IAPB)</strong> under WHO VISION 2020 , The Right to Sight , Global initiative. This year the theme is ‘ <strong>Hope in Sight ’</strong></p>
<p style="text-align: justify;">More than 75% of visual impairment is avoidable. After Cataract, Refractions Errors and Diabetic eye disease comes Glaucoma. The blindness due to these major problems can be prevented. <strong>Glaucoma is the leading cause of irreversible blindness in India and at least 12 million people affected and nearly 1.2 million people blind from the disease.</strong> More than 90% of glaucoma cases remain undiagnosed.</p>
<p style="text-align: justify;"><span style="color: #ff0000;"><strong>Glaucoma can be prevented by regular eye check up</strong></span></p>
<p style="text-align: justify;">Early diagnosis and treatment of Glaucoma is the main goal to prevent blindness. This world sight day, the main focus is on the awareness of importance for <strong>regular eye check up in people above 40 years ,</strong></p>

<li>Family history of Glaucoma</li>
<li>History of seeing Haloes around lights</li>

<p style="text-align: justify;">A slit lamp examination, applanation tonometry, visual field testing and dilated fundus examination are the key tests to be undergone periodically for prevention of blindness. <strong>Glaucoma can be prevented if diagnosed early and further progress can be stopped by local eye drops medications, LASER treatment to the eyes or surgical interventions.</strong></p>
<p style="text-align: justify;">Therefore, the public are requested to take this initiative of  <strong>‘Hope in Sight’</strong> on this world sight day by IAPB and get their eyes examined for Glaucoma and other eye disease for early detection and prevention of the problem. Though COVID19 cross infection is really a major challenge for patients with other illnesses,delaying treatment is not the solution.</strong></span></p>

They were hit hard by the lockdown imposed across India in April and May unable to move out for treatments, no proper guidance from the doctors, not enough attention was paid towards ill patients as they stayed indoors. They have not only been hit hard by the lockdown but the effects of it were very damaging for them in many other ways. But it was mandatory for the government of India to impose restrictions in the larger interest of the people of the country and we must obey the guidelines with no other alternative option.</p>
<p style="text-align: justify;"><span style="color: #ff0000;"><strong>Fears of cross-infection of COVID19:</strong></span></p>
<p style="text-align: justify;">From June onwards the government has started relaxing the restrictions which helped out some serious patients with various illnesses to undergo treatment and take recourse of the doctors and their advice. Still, <strong>the majority of the patients whose condition is aggravating further are reluctant to visit the hospitals with the fears of cross-infection of COVID19.</strong> This may be true to some extent but in another way, it may be fatal as well. Keeping in view of the fears of the patients majority of hospitals are carrying out diligent infection control practices and protocols in the pandemic in all the hospitals. Take an example of MaxiVision eye hospital which has seen an estimated 50,000 visitors to 14 branch hospitals from 1<sup>st</sup> of June till 31<sup>st</sup> July out of which 20,000 are patients with various eye problems. No single visitor got a cross-infection of COVID19.</p>

<p style="text-align: justify;"><img decoding="async" class=" wp-image-4556 alignright" src="https://healthvision.in/wp-content/uploads/2019/11/diabetes-and-eye.png" alt="diabetes-and-eye" width="346" height="283" />There are certain <strong>eye problems like glaucoma and diabetic retinopathy which are the cause of irreparable/permanent blindness for many in India if not treated in time.</strong> Glaucoma is a group of eye diseases that can cause<strong> vision loss and blindness </strong>by damaging a nerve in the back of your eye called the optic nerve.  The symptoms will begin slowly that one may not notice them. The only way one can find out if they have glaucoma is to get a comprehensive eye exam done.  <strong>There’s no cure for glaucoma, but early treatment can often prevent the damage and protect your vision.</strong> Depending upon the severity of the case doctors may advise medicine or laser treatments or Surgery.</p>
<p style="text-align: justify;">As per the reports there are approximately <strong>11.2 million persons aged 40 years and older with glaucoma in India.</strong> Primary open-angle glaucoma is estimated to affect 6.48 million persons. The estimated number with primary angle-closure <strong>glaucoma</strong> is 2.54 million. Those with any form of the primary angle-closure disease could comprise 27.6 million persons. Most of those with the disease are undetected and there exist major challenges in detecting and treating those with the disease.</p>
<p style="text-align: justify;"><strong>Another cause of permanent blindness is Diabetic Retinopathy</strong> if not treated in time. Diabetic retinopathy is caused by damage to the blood vessels in the tissue at the back of the eye (retina). Not properly controlled or poorly controlled blood sugar is a risk factor. Early symptoms include blurry vision, dark areas of vision, and difficulty recognizing colors. It may lead to blindness . Mild cases may be treated with careful diabetes management. Advanced cases may require laser treatment or surgery. There are millions of patients with diabetic retinopathy in India.</p>
