To prevent peripheral neuropathy manage blood sugar level, which involves: anti-diabetic medication, diet control and exercise. And how to manage peripheral neuropathy in diabetes? Key consideration to manage peripheral neuropathy is personal hygiene, blood sugar control and foot care

For a long time now and well in to the foreseeable future, diabetes and its complications are very crutial. A diabetes assumes epidemic proportions. Complications of diabetes like neuropathies especially, peripheral neuropathy is the one that often seeks urgent attention.

Neuropathy, Diabetes means harm to the nerves. It can become a painful condition. It is very frequently found in individuals with persistent high sugar level.

While high levels of blood sugar is well defined, but in practice, we have found patients with 180-200 mg% blood sugar levels seem to console themselves and some feel convinced that they are in good control of the situation. Generally patients don’t comprehend the significance of approaching doctor for regular checking of blood sugar levels.

We have come across people having same medicines for over 5-6 years and still are diabetics over 150 mg% of sugar levels. Educating patients may be a 50% solution in its management.

The single biggest rule

To prevent peripheral neuropathy, work with your doctor and manage your blood sugar level. It is very important to remember that, good management of blood sugar level involves, necessarily a mix of three dimensions, namely: anti-diabetic medication, diet control and exercise.

In general diabetes have influence on nerves, the ones that needs attention is peripheral neuropathy. It usually infects feet and legs. Occasionally it may also infect the arms, stomach and back.

Symptoms

Burning sensation – typically in the evening and night

Tingling/Numbness usually starts and occurs intermittently. It is to be noted that, prolonged period numbness may become permanent and create further complications like wounds and ulcers. It may even become worst because of its painless nature.

Pain occurs commonly in feet and it can be relieved, if blood sugar is controlled and medicated.

Pathophysiology of Diabetic Neuropathy

The important impact of long term diabetes with uncontrolled blood sugar levels are

Neuronal – Diabetes causes patchy damage to the Myelin Sheath

Vascular changes which primarily reduces flow of blood, due to occlusion of micro circulation.

It has its impact on body’s ability of flushing out accumulated biochemical waste and also decreases the want of nutrient supply and infects its normal working.

Key considerations in management of peripheral neuropathy in diabetes

Personal Hygiene

Examine toes, feet and legs everyday

Moisturize if skin is too dry

Take care of your toe nails

Protect feet with well-fitting shoes both at indoor and outside. This is important to prevent injuries and consequent complications like chronic ulcers, non healing wounds and even gangrene

Blood Sugar Control

Primarily blood sugar management has three very important components. Medication, Diet management and Activity/Exercise.

The balance between all three dimensions is essential. Any one of them cannot be stretched infinitely. Many attempts fail miserably when one totally depends on any one dimension.

Foot Care

Use shoes that fit well regularly, so that your feet don’t get injured.

A Podiatrist or Diabetic Foot Care specialist can help in case of complications.

Practically one of the biggest challenge is the time lost in experimenting self-medication, altering diet and activity, very unscientifically.

Management of peripheral neuropathy & chronic ulcers in diabetes

Cold Laser Therapy treatment for peripheral neuropathy evolved in the last decade. It has been used very successfully in managing chronic ulcers, peripheral neuropathy, and burning sensation in the feet. Patients with chronic, non-healing ulcers for quite a few months may observe the development of granulation tissues. Cold Lasers decreases the necessaries for separating gangrene. It has been used successfully to alleviate symptoms of peripheral neuropathy in diabetics.

Cold lasers deliver two significant benefits:

Stimulate and repair myelin sheath

Improve micro-circulation

Cold Laser Therapy is a simple, very sophisticated treatment methodology with no side effects, pain or discomfort.

Do you have Heel pain? Heal it with Cold laser Therapy – the best quality of repair and relief in the short period of time as compared to all other modalities.

Heel pain is a common problem experienced at the bottom of the foot, typically at the point where the heel bears the maximum body weight while standing. It is most painful while trying to get out of bed or after prolonged rest. Symptoms tend to become more manageable after a few steps and continued walking. Heel pain is usually not associated with other neurological symptoms like tingling, burning etc. Overusing the heel in acute pain can also result in swelling and redness.

There are a host of reasons that can cause of heel pain. The three most common causes of heel pain are:

Heel Spur (or Calcaneal spur)

A Heel Spur (or Calcaneal spur) is characterized by an exostosis (bony outgrowth) from the bottom or the back of the heel. An X-ray will confirm the presence of a spur. Not all spurs are symptomatic and may not interfere with routine activities whatsoever.

Constant stress on the foot, usually encountered as a result of the person’s occupation, typically sportsmen, policemen, teachers, catering staff, hospital staff etc. Causing the bony outgrowth and the symptom as described above.

Plantar fasciitis

Each foot has a band of tissue called the ‘fascia’ extending from the heel to the toes. Due to strenuous activity, this band can develop patchy tears and places causing pain and inflammation. This inflamed condition was originally thought to have been caused by heel spurs. Increasingly, the belief has now shifted to heel spur being an aftermath of fasciitis and not the other way around as perceived earlier.

Radiological investigation may reveal nothing or may be a presence of a spur. It is quite possible that a patient may have evidence of a spur but that may be asymptomatic and become somewhat confusing because of the similarity in the symptoms.

Achilles Tendinitis

Achilles tendon is at the back of the ankle. It is the tissue that connects the calf muscles to the heel bone. This tendon may get injured usually due to excessive activity and cause severe pain due to rupture of the tendon fibres. Ankle especially heel movements become painful and may even occasionally cause stiffness. Pain is usually slow onset and increase gradually.

Certain lifestyle related risk factors and use of long term medications are also suspected to contribute to worsening this condition.

Management and treatment modalities

In general for all three conditions (Specific measures are prescribed for each condition and must be followed as advised by the physician).

Rest: Importance of rest cannot be overemphasised. However, certain prescribed activities can help reduce pain. It is important to strike the right balance between rest and activity.

Change in Footwear: Some patients may find wearing shoes or sandals with softer insoles offer good relief.

Cold Compress: Ice packs are very potent way to reduce inflammation, pain and relax the surrounding tissues.

Medication: Typically non-steroidal anti-inflammatory drugs are adequate. However blatant misuse of steroids to avoid frequent trips is quite common.

Physiotherapy: The balance between rest and activity is key to maintaining a normal lifestyle.

Cold Laser Therapy: Cold Laser Therapy is the most definitive therapy for all three conditions. It very effectively and permanently repairs tissues and provides good, long term pain relief.

Surgery: Surgical intervention has limited value and usually considered only for trauma related tissue damage.

Cold Laser Therapy in Heel Pain

All three conditions respond quickly and definitively. Cold laser Therapy offers the best quality of repair and relief in the shortest period of time as compared to all other modalities. In addition to symptomatic relief, cold laser therapy does not have any other significant side-effects commonly associated with use of medication, drugs or surgery. Before and after X-rays (wherever relevant) provide clear evidence of problem resolution. Any need for surgery must be assessed only after considering all non-surgical options unless mandated by any emergency.

Interview with Dr S Raghunandan Founder – CMD & CEO. Dr. Nandan’s Cold Laser Therapy – “People who have lost hope come to us”.

Having treated few thousand patients since inception in 2008, Dr Nandan’s Cold Laser Therapy has pioneered use of cold laser technology in India. They provide well crafted and designer cold laser therapy for Migraine, Stress Headaches, Sinusitis, Cervical Spondylosis, Frozen Shoulder, Slip disc, Sciatica, Knee joint pain, other types of Arthritis, Diabetic Neuropathy, Fibromyalgia, TMJ pain, Heel Spur, Radiculopathy, Nerve Compression, Psoriasis, Acne, Non Healing Ulcer, Skin Allergy Sports injuries, Post Surgical rehabilitation, Sutures, Birth marks, Disturbed Sleep plus 80 medically treatable conditions. They have completed 10 years Cold Laser operations in India as 3 June 2018.Thus helping people across the human world who has lost hope.

Tell us about Dr Nandan’s Cold Laser therapy

Dr Nandan’s Cold Laser Therapy is the pioneer and market leader in India. Generally in any traditional health system, medicines are used to heal or reset imbalances in biochemistry or energy fields of human body. At Dr Nandan’s Cold Laser Therapy, therapeutic lasers are used to heal the pain or tissue damages at tissue or cellular level. Cold laser therapy is a part of regenerative and natural healing system.

When there is an imbalance in internal biochemistry, medicines administered can produced certain side-effects, some of which can be damaging more than promote healing. In cold laser therapy, energy is used to reset internal biochemical ecosystem of glucose, oxygen, enzymes etc. This restoration of the normal internal ecosystem eliminates any existing root causes for degenerative changes and pain. By understanding the problems and its root causes, cold laser therapy can reverse the changes for various problems. The energy dosage planning for each tissue or organ is customizable to individual patient needs for maximum benefit.

Pain relief is achieved through elimination of root cause and not by managing pain signal pathways as in several other alternative systems of medicine. It is aimed at restoring the healthy functioning of underlying organ or system. Our lasers work by supplying energy to the body in the form of billons of photons of light energy. The body absorbs this laser energy on a cellular level and transform it into chemical energy, which the body then uses to commence its own tissue repairs. The stimulating effects of cold laser therapy cause a decrease in inflammation and pain an increase in tissue regeneration and healing. Our therapeutic lasers activate all three known cellular pathways- Adenosine Triphosphate (ATP) pathway for accelerated healing; Nitric oxide (NO) pathway to decrease inflammation; and Lipid Absorption pathway to remove pain.

When did you start the centre and how has the response been?

We are pioneers in bringing this technology to India. When we started ten years ago, it was a totally new concept for anybody to believe it. Most of our initial patients were those who had tried all options like medical, surgical and other treatments, but could not get any relief. These included patients with severe arthritis, acute psoriasis, drug addicts in hopeless condition and list goes on. Many patients who were brought to our centers in wheelchairs or stretchers were able to walk on their own after a few sessions. We addressed several cases of sports injuries, old hair line fractures, respective strain injuries, arthritis, sciatica, spondylitis, tail bone fractures, disc bulge, sinusitis, migraine, wounds, gangrene, diabetic ulcers, diabetic neuropathy, asthma, bronchitis, etc.

How effective is your de-addiction treatment?

Our de-addiction treatment is not only effective but also long lasting because it addresses the body’s craving as well as withdrawal symptoms. Therapeutic lasers rebalance brain chemistry and promote a feeling of relaxation. Many of our patients came out of addiction and presently running successful businesses.

Tell us about your weight reduction solutions?

Our weight management solutions include lasers and other hi-tech machines. A patient can know the progress of his or her treatment by knowing how many calories he or she has lost. Our therapy focuses on two vital areas. It stops excessive conversion of food into fat tissues and it also helps proper utilisation of food intake for energy. In many so called weight management programmes available in market, the focus is only on reducing weight. In this progress, the patient feels low and lacking in energy. In treatment with therapeutic lasers, the body never feel any lack of energy and patients lose weight in a natural and healthy way.

How many centers do you currently run and what are your plans of expansion

At present we have our centres in Malleshwaram , Basavanagudi, JP Nagar, K.G.F, White Field, Diabetic Club, Skin Care Lounge, Medical Director, Quit addictions support care. Very soon, we are going to expand to Indiranagar, R R Nagar, Whitefield, Chennai and Hyderabad in this year. We are planning mobile units to serve the needy, bedridden or old aged patients at their door step. We are associated with world renowned surgeons and doctors from USA, Canada and other developed countries
