Oral Mucositis

After the chemotherapy, radiation therapy for cancer, one of the unfortunate consequences is the development of painful mouth sores, known as oral mucositis. Oral mucositis has emerged as one of the most debilitating side effects of cancer therapy.

Causes of Oral Mucositis

High dose chemotherapy, and localized high dose radiation therapy to the head and neck region are the reasons for developing oral mucositis. These treatments effectively target the rapidly dividing cancer cells, but also inadvertently affect normal healthy cells that rapidly turnover, such as the oral mucosa lining of the inside of the mouth and compromise the integrity of the oral mucosa.

Minor trauma to the mouth from speaking, chewing, and swallowing, is sufficient to breakdown the mucosa, resulting in the formation of painful ulcerations. Hence patients are advised to eat a soft or liquid diet. Mucositis is not an infectious process and therefore cannot be prevented with antibiotics or antiviral medications

Impact of Oral Mucositis

Symptoms of discomfort and pain often precede the visible redness in the mouth and throat. The red areas soon progress to form painful ulcerations. The insides of the cheeks and lips and the sides and underside of the tongue are the most commonly affected sites that causes severe pain and interfere or prevent with eating and drinking. 

The risk of developing mucositis is variable and dependent upon the intensity of therapy, and the patient’s overall health. The most common infection encountered in these cases is a fungal infection. The severity of oral mucositis varies from patient to patient and can last for a few days to in severe cases several weeks to few months. Cadexomer iodine  in wound care management

While there is little data on the choice of appropriate dressings to manage diabetic foot ulcers, research has shown that Cadexomer iodine has superior action compared to many other dressings in chronic infected wounds.² For centuries, iodine-containing products have been utilised as an antiseptic for the treatment of wounds. Historically, iodine was available and used in the form of tincture iodine & alcoholic/aqueous based solutions, which were associated with unpleasant side effects like pain, irritation, and skin staining. The newer iodine preparation in India, Cadexomer iodine, renders additional benefits compared to older iodine formulations with proven safety.

Cadexomer iodine is an antimicrobial dressing ointment with dual benefits. It absorbs all the unwanted fluids from the wounds and in turn, releases iodine slowly which ensures antiseptic action for a prolonged duration. Cadexomer iodine promotes a clean wound healing environment, clears infection effectively, minimizes pain, and accelerates healing. It also has an economic advantage, since it reduces the number of dressing changes required unlike older iodine versions and decreases the overall cost of therapy.

Cadexomer iodine is a newer entrant in wound care management and appears to be a better option in healing ulcers especially in diabetic patients with chronic wounds and biofilms.

What is the treatment for ‘Non-Healing Diabetic Foot Ulcers (DFUs)’? The use of platelet-rich plasma, which is rich in multiple growth factors and cytokines is gaining popularity.

Diabetic foot ulcers (DFUs) are among the most common complications of diabetes mellitus with a lifetime incidence of up to 15% among the diabetic population. Studies have shown that up to 80% of patients with DFUs suffer from both limb ischemia and peripheral neuropathy simultaneously. These conditions further delay healing of DFUs, predisposing to higher rates of complications such as Cellu-litis and Osteomyelitis.

In spite of the high prevalence and morbidity associated with DFUs, current treatment options are limited. Current standard management consists of surgical debridement followed by frequent dressing changes with tight infection and glycemic control. Despite this comprehensive approach, complication and amputation rates remain high.

Among the different methods to achieve ulcer healing, the use of platelet-rich plasma, which is rich in multiple growth factors and cytokines and may have similarities to the natural wound healing process, is gaining in popularity. Contained within these platelets are a number of hemodynamically active proteins that aid in the natural process of wound healing.

Specifically, the platelet alpha-granules contain several of these molecules, including: platelet derived growth factor (PDGF), TGF-β, vascular endothelial growth factor (VEGF), epithelial growth factor (EGF), fibrinogen, fibronectin, and vitronectin. In addition, platelet delta granules contain serotonin, histamine, dopamine, calcium, and adenosine, which act in tandem with the aforementioned growth factors to regulate wound healing.

1 ml of PRP was aspirated and activated with 10% calcium chloride (0.3 ml for 1 ml of PRP) and injected to the ulcer and the edge of the ulcer and covered by paraffin gauze and sterile gauze. The dressing was covered by Dynoplast®. The dressing was opened on the third day and PRP was repeated once weekly. After 1 week, there was reduction in area and the volume of the ulcer to 12.5 cm2 and 3.75 cm3. After six sittings of PRP, the ulcer healed completely in 7 weeks.

What is HBOT?
