Business responsibility in COVID-19 pandemic world intends all responsible corporate citizens to extend all forms of support to COVID-19 relief efforts. 2021 was a year that showed us the true capability that India has, and how much more we can do in 2022.

The COVID-19 pandemic has had a serious and unprecedented impact across the economies and society in general, particularly in India. 2021 was different in every aspect and held more than its fair share of challenges. 2021 was a year where India demonstrated that our people come first – when lives depended on ensuring taking all necessary precautions. India implemented numerous initiatives towards ensuring the health, well-being and safety of our people.

India reeled under the new coronavirus pandemic situation that disrupted our collective sense of the normal, and also witnessed a crisis of unprecedented nature and effect. The harsh situation continues to remain serious, and therefore, we should continue to support our people and our communities.

Best of human nature at this time

However, India also saw the best of human nature at this time. India continually witnesses magnificent examples of resilience. India’s collective resolve to stand with communities, to inspire hope and healing. India is determined to come out stronger from this pandemic than we have ever been. As a sustainable and responsible India, everything we do is to inspire by a deep sense of purpose.

Our collective values have held us in good stead at this time, and they are the foundation on which we have built our success and results. It is heartening to see India rise to the challenge and continue to make a positive impact on the lives of people. In India, the pandemic situation continues to remain serious, and therefore, we continue to support our people and our communities.

India proved the solidity of the Covid-19 strategy. This has helped India quickly adapt to new ways of being stronger together and gain greater momentum addressing key challenges. Government responses in the year 2022 continue to stand robust, both economic and in terms of meeting and exceeding people’s expectations.

India’s COVID-19 vaccination coverage has exceeded 150.61 Cr on January 8, 2022. India is accelerating the pace and expanding the scope of COVID-19 vaccination throughout the country. The vaccination drive has been ramped up through the availability of enough vaccines, advanced visibility of vaccine availability for enabling better planning and streamlining the vibrant vaccine supply chain.

We need to have put in place multiple task forces for people’s hospitalization, medication and vaccination. We need to expand the scope of our employee benefits from medical and term insurance to provide for the families of employees we lost to this terrible virus. Throughout this challenging period, India responded very quickly to prioritize the health and safety of our people and continued to provide a reliable supply of medicines, vaccination and necessary help to the patients across the country.

Nationwide vaccination drive

As part of the nationwide vaccination drive, India has given COVID Vaccines free of cost to people. In the new phase of the universalization of the COVID-19 vaccination drive, India will procure and supply (free of cost) 75% of the vaccines being produced by the manufacturers in the country to various states.

The Indian business cemented its strength with its persistent effort to enhance profitability by improving upon the overall quality of the business. During the year 2022, India needs to continually launch new initiatives and economic relief to Covid-19 affected people and businesses. While India has performed well in the year 2021, It has started 2022 on an optimistic note.

People, civil society and businesses, who collectively enable India to create value by helping each other, will keep focusing on India’s strategic priorities and leveraging their strengths. Trust, confidence, and support to government efforts toward Covid-19 are key for overcoming challenges. Notwithstanding the challenges of the times, Indian businesses should continue to think from the heart and act with a high head – to care for people, covid-19 patients and communities.

Shaped by the COVID-19 pandemic, the unexpected and overwhelming year of 2021 that went by affected every aspect of our lives. In the face of this unprecedented volatility, India made proactive investments in people, processes, operations and technologies that tackle the Covid-19, so that India could deliver strong operational performance throughout the year and into the 2022 and future.

Throughout 2021, India worked hard to strengthen the long-term value of people and business, to emerge stronger coming out of the pandemic. This has been possible due to the hard work, perseverance, and commitment of India’s people. Our priority for the year 2022 will remain to ensure the health and safety of our people. Let’s put in place extensive measures to protect our people and their families, yet sadly we lost several of our valued fellow citizens.

All responsible corporate citizens must continue to extend all forms of support to COVID-19 relief efforts. In a year of turmoil, Indian corporations should be proud and humbled to have been able to serve our communities and needy people. The year 2021 was a year that showed us the true capability that India has, and how much more we can do in 2022. I am very excited about the opportunities ahead for our people and business.

Institute for Health Metrics and Evaluation (IHME) forecasts enormous spread of covid 19 Omicron variant globally. IHME, an independent global health research centre at the University of Washington has released a research report on Omicron, a variant of SARS-CoV-2. Comparing the Omicron variant with the Delta variant IHME research says that the Omicron is more transmissible and with 40-60% immune escape combine to lead to rapid increases in reported case counts. A much larger fraction of infections is symptomatic – likely up from 40% for Delta and prior variants to 90-95% for Omicron.

However, it also shared that based on the best available data, the infection-hospitalization rate is dramatically lower (note the infection-hospitalization rate is not the case-hospitalization rate), likely 90-96% lower than for Delta. The infection-fatality rate is dramatically lower, likely 97-99% lower than for Delta. The research also forecasts around 3 billion infections to occur in the next 2 months. That’s as many infections as was seen in the first 2 years of the pandemic. Peak transmission should be in mid-January with over 35 million global infections a day, nearly three times the Delta wave peak in April.

Given the immune escape and transmissibility, eventually Omicron will reach all country soon, shares the IHME research report. Even countries with strict border controls like New Zeeland based on the Delta experience are likely to eventually see Omicron surges in all country including China we expect in the future. Detected infections at the global level will reach 3 times previous peaks. In the US, the peak will be over 400,000 a day.

The analysis by IHME shows that increasing mask use to 80% will have a huge effect on cases, hospitalizations, and deaths. More rapid and extensive delivery of third doses of vaccine will also reduce the burden of hospitalization and death. The unvaccinated and never infected are at greatest risk. Vaccination for the unvaccinated is a powerful way to reduce personal risk.

Given the rapid spread and increase in Omicron and the high fraction asymptomatic combined with much lower infection-hospitalization and infection-fatality rates, employers and schools will need to re-evaluate their approach to testing and quarantine. Going forward, tracking hospitalizations rather than reported cases will be a more relevant measure for local action than case counts.

There is huge uncertainty particularly about the severity of Omicron which has a critical impact on the forecasts of hospitalization and death. More data in the next 3-4 weeks may change the scenario.

How is Omicron different?

More transmissible and with 40-60% immune escape combine to lead to rapid increases in reported case counts.

A much larger fraction of infections are symptomatic – likely up from 40% for Delta and prior variants to 90-95% for Omicron

Based on the best available data we have, the infection-hospitalization rate is dramatically lower (note the infection-hospitalization rate is not the case-hospitalization rate), likely 90-96% lower than for Delta.

The infection-fatality rate is dramatically lower, likely 97-99% lower than for Delta.

How has our model changed?

To understand Omicron and to reflect timing of past infections, vaccinations and third doses, we have substantially revised our model.

Waning of immunity preventing infection and the slower waning immunity for preventing hospitalization and death are explicitly incorporated in the model.

Waning of vaccine-derived immunity is brand specific, Moderna has the slowest waning curve.

Waning of infection-derived immunity is slightly slower than vaccine-derived immunity.

We explicitly model ancestral, Alpha, Beta, Gamma, Delta, Omicron and other variants including the matrix of protection each type of infection gives to each other variant.

We expect a huge increase in infections.

The Omicron variant is highly transmissible. We project around 3 billion infections will occur in the next 2 months. That’s as many infections as we saw in the first 2 years of the pandemic.

Peak transmission should be in mid-January with over 35 million global infections a day, nearly three times the Delta wave peak in April.

Given the immune escape and transmissibility, we extent Omicron to eventually reach all countries. Even countries with strict border controls based on the Delta experience are likely to eventually see Omicron surges.

Detected infections (reported cases) will increase but not as much as infection.

Detected infections at the global level will reach 3 times previous peaks. In the US, the peak will be over 400,000 a day.

Reported cases (detected infections) will increase less than infections, because we expect the much higher fraction asymptomatic will lower the infection-detection rate.

Hospitalizations and deaths will increase much less than reported cases

At the global level, hospitalizations and deaths will be lower than the prior Delta wave or Northern Hemisphere winter peak.

For the US, hospitalizations and deaths will be below the Delta peak in September and well below the winter 2020/21 peaks.

Policy interventions

Our analysis shows that increasing mask use to 80% will have a huge effect on cases, hospitalizations and deaths.

More rapid and extensive delivery of third doses of vaccine will also reduce the burden of hospitalization and death.

Given the rapid spread and increase in Omicron and the high fraction asymptomatic combined with much lower infection-hospitalization and infection-fatality rates, employers and schools will need to reevaluate their approach to testing and quarantine.

In the future, tracking hospitalizations rather than reported cases will be a more relevant measure for local action than case counts.

Individuals can protect themselves

The vaccinated getting a third dose will substantially increase protection.

The unvaccinated and never infected are at greatest risk. Vaccination for the unvaccinated is a powerful way to reduce personal risk.

Mask wearing remains a very effective strategy to avoid omicron transmission and infection.

For those at increased risk due to age or comorbidities, avoiding indoor gatherings can further reduce risk.

Huge uncertainties

There is huge uncertainty particularly about the severity of Omicron which has a critical impact on the forecasts of hospitalization and death. More data in the next 3-4 weeks may change our view.

Our more severe scenario provides a possible trajectory of data emerges suggesting the IFR and IHR is higher than the ranges we have used in our reference scenario.

Based on 2020, however, data collected during the last 10 days of December and first days of January may be substantially biased. More clarity will likely have to wait until the second week of January.

Currently the Omicron factors we are watching most closely are fraction asymptomatic, degree of immune escape, and severity.

Disposable mask could be the next biggest hazard. Mask wearing reduces 60% risk of COVID-19 infection but dispose the face mask properly.

The prevailing global pandemic has brought everything to a standstill. Ever since the COVID-19 pandemic has begun, there has been an increased need to maintain hygiene. With everything coming back on track with the graded unlocking phases, mask has become a pivotal part of our lives. Face Masks are extremely essential to prevent the spread of Covid-19. Mask wearing reduces 60% risk of COVID-19 infection.

Wearing mask scores over other preventive public health measures from COVID-19 infection as nose and mouth are the main gateways for the entry of the virus. Everyone stresses upon the importance of wearing the mask correctly to prevent the infection and there has been a surge in the demand of the facemask. Frontline COVID Warriors and people employed in the essential services have relied on plastic-based, single-use personal protective equipment (PPE) like masks and gowns to shield themselves from the coronavirus.

Dispose the used masks, gloves properly

The result has been a quickly growing mountain of plastic waste. Gloves & used masks are being disposed off wherever possible as people are not aware of the risks it can carry. It is important that we dispose the used masks, gloves properly and help avoid the spread of infections. Some people are picking up the used masks and reusing them which is putting their life in danger.

According to a study 75% of the estimated mask could end up in landfill or the sea. Discarded single-use face masks used to stop the spread of coronavirus could be causing significant harm to the environment. Most of these masks contain or are made of polypropylene, which does not break down quickly. Marine plastic pollution is a serious problem.

It is estimated that every year, over eight million tons of plastic enter our oceans. This plastic does not disappear but rather slowly breaks down into micro-plastic, which enters food chains, with devastating effect. As the pandemic is likely to drag on, we should address these plastic pollution issues immediately.

It is better to use reusable cloth masks that can be washed, dried and reused.

Each person should have their own masks and not share among family members.

Decontaminate the used masks, gloves etc, with sanitizer, cut them and dispose them in separate bags and not mix them with household stuff.

The disposed masks, gloves could be a potential source for the spread of the deadly contagious virus and should be treated as medical waste. But there is no mechanism for collecting and disposing of these currently.

Home isolation programme can be considered a boon for Covid-19 patients with mild symptoms says Union Ministry of Health & Family Welfare’s report.

The traditional model of hospital-based healthcare delivery in India has always been scarce. And the spurt in the cases of COVID-19 has significantly increased the burden on the limited existing healthcare infrastructure in the country. However, the pandemic has also changed the dynamics of the healthcare delivery chain in India as 80 per cent of COVID-19 patients are asymptomatic or showing mild symptoms, reveals a report of Union Ministry of Health & Family Welfare.

The government recommends home isolation:

These patients are not required to go for hospital-based treatments and in this regard, the guidelines of the Indian Council of Medical Research say that persons clinically categorised as having very mild or pre-symptomatic (COVID-19) will have the option of home isolation provided they have the requisite facility at their residence. Understandably, the government recommends home isolation with the intent to ease the burden on the hospitals.

Thanks to the government guidelines, we have been providing home isolation services to Fortis Hospital across the country, and also collaborated with the corporate like Tata, Dabur, ABFL and insurance companies for our HIP to manage the asymptomatic as well as the patients with mild symptoms. We have developed customised Home Isolation Programme (HIP) under two caps — ‘COVID Essentials’ and ‘COVID Advanced.’ The former is meant ‘for low-risk individuals’ and the latter ‘for high-risk individuals. Home isolation is feasible and affordable for everyone. And, this will also decrease the burden on hospitals. As the number of mild and asymptomatic COVID-19 patients has been increasing gradually, it seems that 90% of the patients will be treated through home isolation care considering its affordability and feasibility.

We have been receiving overwhelming responses from patients amidst the COVID-19 panic as they are approaching us considering the feasibility of Home Isolation Programme (HIP). Two weeks ago, one of the perplexed patients with mild symptoms of COVID-19 reached out to our helpline, who was painstakingly responded by our HIP team. He was immediately connected to the concerned physician followed by a psychologist to ease his mental stress and a dietician. Subsequently, testing was done via hassle-free sample collection at home. Rest, our HIP team has managed remotely with the fast recovery of the patient.

Queries for home isolation has increased significantly :

In the last three months, the number of queries for home isolation has increased significantly as we are receiving 250-300 queries per day. So far, we have treated over four thousand patients through our HIP. To start the HIP, first, we do on boarding call with the patients and their family members by trained COVID-19 experts on how to implement HIP protocols. We provide complete training and education ‘On how to live in home isolation’ with IEC material. Caregivers are also available round the clock.

Amid the chronic shortage of hospital beds almost everywhere, home isolation services turn out to be the most preferred and feasible choice. Recently, the UP government has decided to allow home isolation for asymptomatic COVID-19 patients under certain conditions. Regarding the allowance of home isolation for asymptomatic COVID-19 patients, Additional Chief Secretary (Home), Awanish Kumar Awasthi ha said, “So far, home isolation was not allowed in the state. But now we have decided to allow it with certain conditions and norms. He also added that there is an adequate number of beds in COVID-designated hospitals in the state, and the decision to allow home isolation was made based on ‘requests from people’.”

COVID testing, logistical challenges and how to address them is a major problem especially in developing countries. Alternative testing protocols that are equipped to allow testing with limited resources will be incredibly useful.

As COVID-19 deaths and hospitalizations keep mounting every passing day, adequate testing has become a pressing issue. Though conversations around “need for adequate testing” have been on the rise in the past few months unfortunately, majority of these conversations do not consider some critical issues around test characteristics, test availability and most importantly, test strategy.

The infrastructure needed to detect SARS-CoV-2 infection that adequately complies with WHO guidelines is lacking across many parts of the globe, especially in developing countries. The problems faced are many and hence, there is an immediate need for the national and international communities to collaborate in the development of novel protocols for rapid detection of COVID-19 to fight effectively against the outbreak.

It is crucial to address the issue

According to a report by Science Direct, while the majority of people with COVID-19 develop a mild illness, around 14% develop severe disease requiring hospitalization, and 5% need ICU support. Inadequate medical facilities to patients in a community can lead to more disease transmission from untreated cases. WHO has strongly recommended testing of both the suspects for COVID-19 and those who have been in contact with them to curb the chances of infection in a community.

However, due to the lack of appropriate resources like testing laboratories and kits, we are facing an increasing number of fatalities while struggling to contain the spread of viruses. The WHO has recommended using a reverse transcription polymerase chain reaction (RT-PCR)-based protocol for testing COVID-19 infection. However, it involves a huge cost as it requires RNA extraction kits, costly RT-PCR machines, and trained technicians to operate them. The establishment of this facility requires huge expertise, investment and time, which are currently limited by the crisis. Alternative testing protocols that are equipped to allow testing with limited resources will be incredibly useful.

Diagnostic testing: a key issue in India

Diagnostic testing is a key agenda of the policy debate around COVID-19 interventions in India. According to the Center for Global Development, as of June 1, 2020, approximately 3.8 million tests had been conducted in India since it began in February. However, considering the size and needs of the population, it is still drastically insufficient. If we compare daily COVID-19 tests in India with other countries, the number is quite low, 0.08 tests per 1,000 people in India against 1.16 in the US and 1.02 in Italy.

Also, the test positivity rate on the same day stood at about 5% which is within the WHO’s recommended rate of <10%. If we look at the scenario at global level, testing inadequacy has hampered the COVID-19 response in the USA, Europe, and Brazil; it may prove to be an even greater challenge in India. The RT-PCR throat/nasal swab test, as recommended by WHO and ICMR, has been the key diagnostic test used in India. However, the central organisation has recently directed states to conduct serosurveys using an IgG ELISAtest which provides information about the history of the infection as against RT-PCR which detects the presence of a live virus.

The rapid antibody tests can help guide isolation policies and better track people who are immune. Currently, 1,00,00 tests are being conducted in the country every day, which the government aims to double but the reality is that even doubling the number, is not sufficient. If we look at the experiences of the USA and Europe, testing needs to be increased by ten times to achieve a meaningful epidemiologic understanding of the prevalence. Such a large-scale increase would put the testing supply chain’s scaling-up ability to the test as there have already been many reports revealing long turnaround time for test reports, shortages in testing supplies and delays in timely notification. Testing can be termed adequate only when there is a proper interpretation of test results and mandated follow-up actions.

Recommended Actions

1. Build a pragmatic testing strategy:

The most important component of a scaled-up testing strategy is using a combination of PCR tests, antibody tests, and at-home testing. Many countries, including the UK and the USA, are increasingly using antibody tests as they are capable of identifying pockets of infection at a large scale and those with immunity. They will help the government to become more targeted and pace up economic recovery. The pooling of RT-PCR samples has been efficient for surveillance of international passengers and migrant workers in addition to being cost effective and therefore, it has the ability to ramp up testing strategy. Moreover, from the experience of the USA, at-home testing and specimen collection has a great scope in the future.

2. Faster turnaround time for testing:

Rapid turnaround time helps in quicker isolation of confirmed cases and contact tracing and also, it provides more up-to-date and reliable data. Improving TAT requires a more careful analysis of the trade-offs involved. TAT should be publicly reported, like the number of tests carried out daily. This would enable faster detection of testing bottlenecks in addition to enabling their proactive resolution.

3.Re-design the supply chain network:

To scale-up the testing capability, it is important to refine the supply chain strategy and determine the optimal product sourcing and inventory deployment rules. Proactively understanding the possible constraints in advance can help in reconfiguring the supply chain before they slow down the whole system.

Holistic health is instrumental in debarring COVID-19 infection. Strong immunity not only plays pivotal role in combating the infectious COVID-19 but also all-time armour against any ailment.

The significance of holistic health and its role in shielding us from any ailment is universal. Adding stimulus to this perception, our PM, in the course of fighting against COVID-19 pandemic and defeating it, has recently urged the people to follow a Ministry of Ayurveda, Yoga & Naturopathy, Unani, Siddha and Homoeopathy (AYUSH) protocol on ways to stay fit and enhance immunity and make it a part of their lives.

The protocols of the AYUSH Ministry document states that enhancing the body’s natural defence system (immunity) plays an important role in maintaining optimum health. We all know that preventing is good than curing it later. While there is no medicine for COVID-19 so far, it will be wise to take preventive measures, which boost our immunity in these times. It has also advised the usage of turmeric, cumin, coriander and garlic in cooking. Jaggery, fresh lemon juice too can be helpful in the resistance of COVID-19.

Ayurveda, being one of the world’s oldest holistic healing systems is based on a belief that health and wellness depend on a delicate balance between circulatory (Vata), metabolic (Pitta) and matter (Kapha) aspects and perfect coordination between the mind, body and spirit. It aims to promote good health, not just fight disease. And we see, the same mechanism is playing out in the COVID-19 infected cases as the people recovering are due to their strong immunity and not medicine.

Ayurveda’s sweeping awareness on deterrent care derives from the notions of ‘Dinacharya’ — daily regimes and ‘Ritucharya’ — seasonal regimes, healthy diet as per one’s own body-type to maintain a healthy life. The awareness about oneself and harmony with nature can achieve stronger immunity, which has been emphasised across Ayurveda’s classical scriptures.

In the wake of the COVID-19 infection, variations in the symptoms of the people infected have been observed. It has been also found that some people have mild symptoms and some have none, while others, mostly the old and sick have developed fatal pneumonia. But the larger question is why such variations are seen among the infected people, though the infectious virus is the same. There is a simple but very strong reason behind these variations i.e., the difference in ‘immune competence’ among the people and their ability to repulse and overcome the infection. The stronger the immunity, the safer you are — the weaker the immunity, the more vulnerable you are.

Physical distancing, handwashing, using sanitisers and other preventive measures to avoid the exposure of the virus — remain crucial to avoid terrible overthrow of healthcare. However, focusing exclusively on strengthening our immune and body-repair competences can not only slow contagion but completely eradicate too — the most powerful tool to combat this pandemic. Unfortunately, this powerful tool is a serious omission. Even at the post-infection stage administration of immunity-boosting, Rasayanas will help curb the upsurge and will provide a faster cure. Strong immunity is the shield that protects us from every infection — this is what Ayurveda teaches in general.

Ayurvedic viewpoint on COVID-19:

Ayurveda defines health as a perfect balance amongst all the aspects of physiology; Dosha (Vata, Pitta, Kapha), Agni (13 digestive fires), Dhatu (7 body tissues), Malkriya (elimination of all types of waste) and bliss at the level of senses, mind and Atma. Ayurveda does not give much emphasis to the treatment of a disease but on enhancing patient’s ability to re-establish balance and bliss. And this is the basis of Ayurvedic treatment to deal with COVID-19. Present facts also support this. Even though there is no medicine for this viral infection, still, a big percentage of patients are recovering, and also the fact that only those with compromised immunity are unable to get rid of the infection. This clearly shows that if Ayurveda support is given to patients, the recovery could be faster and without much damage to the system.

When proper balance and bliss is established a very fine substance is produced called Ojas. As per Ayurveda, Ojas is one of the most important elements for maintaining and sustaining life. This helps in strengthening balance, purity in thoughts, health and positivity in feelings in every situation, better immunity, longevity, intelligence and memory. This is also known as Bala (strength) because it provides strength to the body in terms of physical, mental, immunological level and resistance to the body.

Summarily, if defence mechanism and self-repair mechanism is strengthened, and if the Ojas is maintained up to the mark, it can fight back any virus and the same mechanism of resistance will apply in the case of COVID-19 pandemic. And to make Ojas lively, we need to follow the natural lifestyle, means Ayurvedic way of living. i.e., ‘early to bed and early to rise’, physical exercise, Yoga, ‘Transcendental Meditation’ and proper diet.

In line with the natural way of life, which is the Ayurveda way of life, it is essential that we must focus on strengthening our immune system and defence mechanism. In this direction, Ayurvedic recommendations, which include diet, daily routine, various Rasayanas, medicines, mental technique, Yogasanas, Pranayama, will be very helpful. It also focusses on the individual rather than on the disease. This is how the immune system, defence mechanism and self-repair mechanism is strengthened.

Understandably, if a person’s defence mechanism is strengthened, no external aggression like Coronavirus or any disease will dare to attack. However, in case of attack, it will subside soon without threatening life as is seen in COVID-19 pandemic, wherein, out of infected people, many have been recovered. So, the takeaway is — the recovered people have strong defence mechanism and immunity — the unfailing armour against any disease.
