Chronic wounds are diseases that haven’t been successfully treated during the process of treatment. The costs associated with chronic wounds are much higher than normal disease treatment. Chronic wounds can result in severe conditions that can be potentially life-threatening like amputation. This will emerge in favor of growth of several chronic wound care centers in the coming years.

Diabetic foot ulcers is a serious complication of diabetes in the long term. India is known as the Diabetes capital of the world. Of the estimated 77 million diabetics in the country, 25% are expected to develop diabetic foot ulcers during their lifetime. Amongst them, nearly 50% would have an infected diabetic foot ulcer which would require hospitalisation, and a staggering 20% would undergo an amputation.

Diabetic foot

Diabetes lowers the immunity of our body to fight a variety of diseases. The foot is a part of the body that generally escapes regular care. When that happens, the skin and tissues of the foot start deteriorating. This deterioration may be exacerbated by an added infection setting in leading to a wound. Such wounds tend to take a long time to heal or resurface after healing, or sometimes just don’t heal at all especially in diabetics and are referred as diabetic foot ulcers.

Diabetic foot is a serious complication of diabetes in the long term and can lead to amputation if left untreated. Hence, diabetic patients should seek guidance from their doctors. Here, early diagnosis and treatment are the keys to manage diabetic foot ulcers effectively. In India, though awareness about diabetes has improved significantly in the past decade, diabetic foot ulcers still lack the required attention.

Improper foot care and poor glycaemic control are amongst the primary reasons for the development of diabetic foot ulcers. These ulcers are often difficult to heal and require long term follow up and treatment. People are unaware and very casual about foot care till it is usually too late which often leads to delay in consultation & treatment.

Types of chronic wounds are

1. Diabetic Ulcers, Pressure Ulcers, Venous Ulcers.

2. Arterial Ulcers, Burns Bedsores, Accident injuries

Modern wound care methods

1. Advanced Wound Dressings with liquid collagen, stem cells, other cellular and Targeted Nutrition products.

2. Active Therapy with Therapeutic lasers HBO2

Chronic wound healing has great market potential

Chronic wound healing has great market potential in medical practice in the coming years. The prevalence of chronic, and surgical wounds has increased significantly due to the growing global geriatric population, rising traumatic wounds, increasing number of surgeries, and obesity and diabetes.

During 2018-19 appx around 5.7 million people in India, suffered from chronic wound diseases. The high prevalence of the chronic wounds will create great opportunities for companies operating in the market. Innovations in the field of Regenerative medicine by GIOSTAR- ReMeD have led to new dimension in chronic wound healing market in India. In 2019 the global chronic wound care market size was $10.12 Bn & is projected to reach $16.36 Bn by 2027. The Asia Pacific, Latin America and Africa are relatively untapped markets for wound management and have huge growth potential.

GIOSTAR (Global Institute of Stem Cell Therapy and Research) which is a world leading Stem cells Institute has a dedicated division under regenerative Medicine. It has an innovative and a unique medical treatment system to manage different chronic degenerative conditions non surgically using new frontiers of healthcare like Targeted Nutrition, Therapeutic Lasers, Metabolic Medicine, Preventive health care and cellular therapy. The significant positive outcome of advanced wound care over traditional wound care are the key factors driving the growth of this market. Regenerative Medicine has been called the next evolution of Medical treatments. Because of its unique Healing potential it is expected to revolutionize chronic Healthcare.

Oral Mucositis

After the chemotherapy, radiation therapy for cancer, one of the unfortunate consequences is the development of painful mouth sores, known as oral mucositis. Oral mucositis has emerged as one of the most debilitating side effects of cancer therapy.

Causes of Oral Mucositis

High dose chemotherapy, and localized high dose radiation therapy to the head and neck region are the reasons for developing oral mucositis. These treatments effectively target the rapidly dividing cancer cells, but also inadvertently affect normal healthy cells that rapidly turnover, such as the oral mucosa lining of the inside of the mouth and compromise the integrity of the oral mucosa.

Minor trauma to the mouth from speaking, chewing, and swallowing, is sufficient to breakdown the mucosa, resulting in the formation of painful ulcerations. Hence patients are advised to eat a soft or liquid diet. Mucositis is not an infectious process and therefore cannot be prevented with antibiotics or antiviral medications

Impact of Oral Mucositis

Symptoms of discomfort and pain often precede the visible redness in the mouth and throat. The red areas soon progress to form painful ulcerations. The insides of the cheeks and lips and the sides and underside of the tongue are the most commonly affected sites that causes severe pain and interfere or prevent with eating and drinking.

The risk of developing mucositis is variable and dependent upon the intensity of therapy, and the patient’s overall health. The most common infection encountered in these cases is a fungal infection. The severity of oral mucositis varies from patient to patient and can last for a few days to in severe cases several weeks to few months. Very little that can be done to prevent or treat the mucositis from occurring. Because of its complex biological nature makes the management of oral mucositis a challenge in cancer therapy.

Stem cells therapy is well known for the regeneration or repair of the damaged tissue or organs. Therefore, stem cell may be a very promising treatment for the oral mucositis. GIOSTAR has developed a proprietary protocol to treat the mucositis using stem cells with the combination with several nutritional and natural substances.

Platelet Rich Plasma (PRP): An alternate for chronic ulcers treatment. Platelet Rich Plasma (PRP) represents a viable alternative treatment for recalcitrant chronic ulcers which do not respond to conventional treatment.

Platelet-rich plasma (PRP) is defined as a portion of the plasma fraction of autologous blood having a platelet concentration above baseline. PRP serves as a growth factor agonist and has both mitogenic and chemotactic properties. It contains a high level of platelets and a full complement of clotting and growth factors. In recent years, the use of platelet-rich plasma (PRP) has increased notably in a range of diseases and settings, such as wound healing, treatment of chronic skin and soft tissue ulcerations, the use of PRP include periodontal and oral surgery, maxillofacial surgery, orthopaedic and trauma surgery, cosmetic and plastic surgery, spinal surgery, infertility, heart bypass surgery and burns.Considering cutaneous ulcers, good outcomes have been found in case series and controlled studies in a wide variety of etiologies.

PRP-solutions

PRP operates as a tissue sealant and drug delivery system, with the platelets and helps to cure wound by freeing locally acting growth factors via α-granules degranulation. These growth factors aid healing by attracting un-differentiated cells in the newly formed matrix and triggering cell division. PRP may suppress cytokine release and limit inflammation, interacting with macrophages to improve tissue healing and regeneration, promote new capillary growth and accelerate epithelialization in chronic wounds.

Platelets in PRP also has important role to play in host defence mechanism on wound by generating proteins that attract macrophages. PRP contains a few leukocytes that makes interleukins as a part of a non-specific immune response. PRP can be considered a secure treatment. No adverse effects, such as increased risk of infection or hypersensitivity reactions, have been detected in clinical trials. Regarding oncogenic potential, when possible coincidences between carcinogenesis and the mitogenic pathways employed by growth factors have been evaluated, no evidence supports a possible tumoral triggering. Once a growth factor has joined its membrane receptor, intracellular signal cascades are activated, normal genetic expression is promoted, and different control mechanisms regulate this process.

Chronic wounds that do not respond to conventional treatment are not rare, and thus constitute a real challenge for the clinician. PRP represents a viable alternative treatment for recalcitrant chronic ulcers, whose efficacy has been demonstrated both in vitro and in vivo. The design of clinical protocols and the extension of PRP use in wound-care centres could have an important socioeconomic impact and see improvement in patients’ quality of life. Regenerative medicine in wound healing is a continuously innovative area. Its objectives are both providing essential elements for damaged-tissue replacement and enhancing intrinsic regenerative capacity. Clinical experience shows that combination of different regenerative techniques such as stem cells may enhance healing.
