<p style="text-align: justify;"><strong>National, May 22022:</strong> In a bid to promote scientific evidence-based Ayurveda across the country, a four-day National Ayurved Cardiology Symposium – first of its kind in India, was held to present and share various case studies supported with clinical data about Ayurveda’s success in treating and reversing cardiac diseases. Madhavbaug, brand of Vaidya Sane Ayurved Laboratories Ltd, a specialist in treating and reversing cardiac related ailments through their Ayurvedic treatment and therapies brought together about 5,000 doctors from across India for this virtual conference, which saw even wide participation from the practitioners of modern medicines.</p>
<p style="text-align: justify;">The Symposium was powered by Medemy.in and MIPC chair of Maharashtra University of Health Sciences was the knowledge partner. Addressing the participants, <strong>Dr Rohit Madhav Sane, MD &amp; CEO of Madhavbaug</strong> underscored the need for scientific evidence based practice of Ayurveda to unleash its real potential. “A major concern about Ayurveda is the lack of evidence based approach in its treatment procedure. Any treatment method to stand the test of time should be scientifically proven. We have taken a major initiative in that direction by developing a standard treatment protocol for over 100 disorders based on clinically validated data,” Dr Sane added.</p>
<p style="text-align: justify;">In her inaugural speech, <strong>Dr Madhuri Kanitkar, Lt. Gen (Retd), Vice Chancellor of <a href="https://www.muhs.ac.in/" target="_blank" rel="noopener">Maharashtra University of Health Sciences (MUHA)</a></strong> made a strong pitch for the proper documentation of Ayurveda’s success in treating various illnesses.</p>
<p style="text-align: justify;">Echoing a similar thought, <strong>Dr Jagdish Hiremath, Chair Professor of Madhavbaug Institute of Preventive Cardiology chair by Maharashtra University of Health Sciences</strong>, said proper scientific record of various Ayurvedic treatment therapies and drugs will help the industry to make a major mark at the global level.</p>

<p style="text-align: justify;"><span style="color: #ff0000;"><strong><sub> </sub><em>A clinical case report</em> </strong></span></p>
<p style="text-align: justify;"><span style="color: #ff0000;"><strong>ABSTRACT</strong></span></p>
<p style="text-align: justify;">PCOS, Fallopian Tubal Blockage, Repeated Spontaneous Abortions are some of the causes of female infertility.  Hormonal treatment or drilling of ovaries, micro-surgery of fallopian tubes, IVF are some of the treatment. In <a href="https://healthvision.in/category/articles/ayurveda/" target="_blank" rel="noopener">Ayurveda</a> it is named as Vandhyatva. Garbhasrava, Beeja Dushti and Mithya Ahara Vihara are some of the causes mentioned by our Acharyas. We are reporting a case of infertility due to PCOS with a surgical history of emergency laparoscopic right sided salpingectomy for ectopic pregnancy. And medical history of tab methotrixate for left side ectopic pregnancy, consequently for second time &amp; left sided hydrosalpinx. As per the classical reference treatment was planned for four- karma (virechana karma, kala basthi and marsha nasya karma, and Uttara basthi karma) along with music therapy and yoga chikitsa, accompanied by rasayana chikitsa, to achieve shuddha garbha sambhava samagri (pure factors of fertilization &amp; conception). Treatment plan was successful with intra uterine pregnancy and she has delivered a healthy female baby through caesarian section.</p>
<p style="text-align: justify;"><span style="color: #ff0000;"><strong> </strong><strong>INTRODUCTION</strong></span></p>
<p style="text-align: justify;">According to the World Health Organization (WHO), infertility can be described as the inability to become pregnant, maintain a pregnancy, or carry a pregnancy to live birth<strong>.<sup>(1)</sup></strong> A clinical definition of infertility by the WHO and ICMART is “a disease of the reproductive system defined by the failure to achieve a clinical pregnancy after 12 months or more of regular unprotected sexual intercourse.” <strong><sup>(2)</sup></strong> Infertility can further be broken down into primary and secondary infertility. Primary infertility refers to the inability to give birth either because of not being able to become pregnant, or carry a child to live birth, which may include miscarriage or a stillborn child. <strong><sup>(3)(4)</sup></strong></p>
<p style="text-align: justify;">PCOS &#8211; Poly Cystic Ovarian Syndrome is a set of symptoms due to elevated androgens (male hormones) in females<strong>.<sup>(5)(6)</sup></strong> Signs and symptoms of PCOS include irregular or no  menstrual periods, heavy periods, excess body and facial hair, acne, pelvic pain, difficulty getting pregnant, and patches of thick, darker, mood disorders, velvety skin<strong>.<sup>(7)</sup></strong></p>
<p style="text-align: justify;">PCOS is the most common endocrine disorder among women between the ages of 18 and 44<strong>.<sup>(8)</sup></strong> It affects approximately 2% to 20% of this age group depending on how it is defined<strong>.<sup>(9)(10)</sup></strong> When someone is infertile due to lack of ovulation, PCOS is the most common cause<strong>.<sup>(5)</sup></strong></p>
<p style="text-align: justify;">PCOS is due to a combination of genetic and environmental factors <strong>.<sup>(11)(12)(13)</sup></strong> Risk factors include obesity, a lack of physical exercise, and a family history of someone with the condition<strong>.<sup>(14)</sup></strong> Diagnosis is based on two of the following three findings: no ovulation, high androgen levels, and ovarian cysts<strong>.<sup>(15)</sup></strong> Cysts may be detectable by ultrasound<strong>.<sup>(16)</sup></strong></p>

<p style="text-align: justify;">PCOS has no cure<strong>.<sup>(17)</sup></strong> Treatment may involve lifestyle changes such as weight loss and exercise.<sup>(18)(19)</sup> Birth control pills may help with improving the regularity of periods, excess hair growth, and acne<strong>.<sup>(20)</sup></strong> Metformin and anti-androgens may also help<strong>.<sup>(20)</sup></strong> Other typical acne treatments and hair removal techniques may be used<strong>.<sup>(20)</sup></strong> Efforts to improve fertility include weight loss, clomiphene, or metformin<strong>.<sup>(21)</sup></strong> In vitro fertilization is used by some in whom other measures are not effective<strong>.<sup>(21)</sup></strong></p>
<p style="text-align: justify;"><strong>Ectopic pregnancy</strong> is a complication of pregnancy in which the embryo attaches outside the uterus.<strong><sup>(22)</sup></strong>.</p>

<p style="text-align: justify;"><span style="color: #ff0000;"><strong>Causes of tubal pregnancy</strong></span></p>
<p style="text-align: justify;">Any sexually active woman of childbearing age is at risk of an ectopic pregnancy and often the reason for the ectopic pregnancy will never be determined. Tubal pregnancy is when the egg is implanted in the fallopian tubes. Hair-like cilia located on the internal surface of the fallopian tubes carry the fertilized egg to the uterus. Fallopian cilia are sometimes seen in reduced numbers subsequent to an ectopic pregnancy, leading to a hypothesis that cilia damage in the fallopian tubes is likely to lead to an ectopic pregnancy<strong>.<sup>(23)</sup></strong> Women who smoke have a higher chance of an ectopic pregnancy in the fallopian tubes. Smoking leads to risk factors of damaging and killing cilia<strong>.<sup>(23)</sup></strong> As cilia degenerate, the amount of time it takes for the fertilized egg to reach the uterus will increase. The fertilized egg, if it doesn&#8217;t reach the uterus in time, will hatch from the non-adhesive zona pellucida and implant itself inside the fallopian tube, thus causing the pregnancy.<strong><sup>(23)</sup></strong> Women with pelvic inflammatory disease (PID) have a high occurrence of ectopic pregnancy.<strong><sup>(24)</sup></strong> This results from the build-up of scar tissue in the fallopian tubes, causing damage to cilia <strong><sup>(25</sup></strong><sup>)</sup>.  If however both tubes were completely blocked, so that sperm and egg were physically unable to meet, then fertilization of the egg would naturally be impossible, and neither normal pregnancy nor ectopic pregnancy could occur.<strong><sup>(25)</sup></strong></p>
<p style="text-align: justify;">A history of a tubal pregnancy increases the risk of future occurrences to about 10%<strong>.<sup>(25)</sup></strong> This risk is not reduced by removing the affected tube, even if the other tube appears normal.  Fertility following ectopic pregnancy depends upon several factors, the most important of which is a prior history of infertility.</p>
<p style="text-align: justify;"><span style="color: #ff0000;"><strong>CASE PRESENTATION</strong></span></p>
<p style="text-align: justify;">A lady with married life of 6yrs from Bidadi in Banglore, presented with amenorrhoea -1 &amp; ½ month, with history of irregular, scanty, painful menstruation. Her past medical history revealed twice tubal ectopic pregnancy.</p>
<p style="text-align: justify;"><span style="color: #ff0000;"><strong>Timeline </strong></span></p>
<p style="text-align: justify;">Lady had developed irregular, scanty, painful menstruation after undergoing IVF (In-Vitro-Fertilization) in one of the Hospital at Banglore in 2017. However, IVF was not successful. Ultra-sonography of the abdomen &amp; pelvis revealed bilateral PCOD (Poly Cystic Ovarian Disease). She was treated with oral hormonal pills for 6moths. During the course of oral hormonal treatment she had regular but painful menstruation. But she did not conceive. Later she visited Ayurveda clinic in Mysore for her problems. Later in June 2018 she consulted our hospital. She underwent ultra-sonography of abdomen &amp; pelvis (August 2018). This report also revealed as bilateral PCOD.</p>
<p style="text-align: justify;">In 2013, she had undergone emergency laparoscopic right sided salpingectomy under spinal anaesthesia, as she was diagnosed very late with right sided ruptured tubal ectopic pregnancy.</p>
<p style="text-align: justify;">In 2015, she had conceived second time, however, again it was left tubal pregnancy, as it was diagnosed early at 5wks, it was terminated by oral pills &amp; was treated with medicine Methotrixate.</p>
<p style="text-align: justify;">After that she underwent HSG (Hystero-Salpingography) &amp; the report showed,</p>
<p style="text-align: justify;"><strong>Left tube –hydrosalpinx in ampullary region, fimbrial end clumped. However, free spill was seen.</strong></p>
<p style="text-align: justify;">After this event she did not conceive.</p>
<p style="text-align: justify;">In 2017, once she underwent IVF (In-Vitro-Fertilization) in Milan Hospital Banglore. However, it was not successful. Since then she developed irregular menstruation &amp; diagnosed with PCOD (Poly Cystic Ovarian Disease) in ultrasonography report.</p>
<p style="text-align: justify;">In June 2018, she came to SDM Institute of Ayurveda &amp; Hospital Banglore. From august 2018 she was regular in taking medicine and councelling.</p>
<p style="text-align: justify;">Menstruation was found to be irregular (50- 90 days usually gets after taking hormonal pills she used to get her menstruation), scanty, painful, 3 – 4 days -dark blackish red, more clots than liquid bleeding &amp; required 1 pad/day, but she changed 2pads/day to maintain hygiene. Her inner garment used to get stained during menstruation.</p>
<p style="text-align: justify;"><span style="color: #ff0000;"><strong> </strong><strong>Diagnostic focus and Assessment</strong></span></p>
<p style="text-align: justify;"><strong>Per Vaginal Examination revealed &#8211; </strong>Uterus- AVNS, vagina was bathed in yellowish – white discharge.</p>
<p style="text-align: justify;"><strong>Per Speculum Examination revealed </strong>– Cervical erosions all over the cervix with severe yellowish- white discharge adhered to the cervix &amp; vagina.</p>
<p style="text-align: justify;"><strong>HSG (Hystero-Salpingography) in 2015 &#8211;</strong> <strong>Left tube –hydrosalpinx in ampullary region, fimbrial end clumped. However, free spill was seen.</strong></p>
<p style="text-align: justify;">After analyzing, with the ayurvedic principles, it was diagnosed as<strong>, </strong></p>
<p style="text-align: justify;"><strong>‘Vandhyatva’</strong> <strong>‘Secondary Infertility’ due to PCOS ( PCOD, Pimples, Hair loss)</strong></p>
<p style="text-align: justify;">In this case, the <strong>six causative factors (Ashuddha garbha sambhava samagri (impure factors of fertilization &amp; conception) &amp; manasika shalyas )</strong> for vandhyatva are explained as follows,</p>
<ul>
<li><strong>Beeja dushti </strong>&#8211; Anovulation due to PCOD,</li>
<li><strong>Rutu dushti </strong>&#8211; Irregular menstrual cycle,</li>
<li><strong>Kshetra dushti </strong>&#8211; Hydrosalpinx in the ampullary region of the left fallopian tube, right sided &#8211; salpingectomy done, cervical erosion.</li>
<li><strong>Arthava dushti (Arthavakshaya,Athyarthava,Kashtarthava) &#8211; </strong>irregular menstruation, sometimes amenorhoea and other time meno-metrorrhgia/oligomenorrhoea, dysmenorrhoea.</li>
<li><strong>Ambu dushti </strong>– Body fluids -All the doshas vitiated (in bhedhavastha of shad kriyakala) and <strong>ama</strong> (free radicals) produced also vitiates body fluids. Her vyadhi kshamatva (immunity) decreased (often she gets rhinitis, cough). Homeostasis was disturbed in the body.</li>
<li style="text-align: justify;"><strong> </strong><strong>Shalyas </strong>&#8211; Stressful factors<strong> (manaha shareera bhadhakarani iti shalyani.</strong>) were present in her. As she was, physically, mentally, emotionally &amp; socially stressed out, due to medical history<strong>. </strong>This leads to disturbance in homeostasis. Lady had entered into flight mode due to stress.</li>
</ul>
<p style="text-align: justify;"><span style="color: #ff0000;"><strong>Therapeutic focus and Assessment</strong></span></p>
<p style="text-align: justify;"><strong>Chikitsa was aimed at objectives ‘A’ and ‘B’</strong></p>
<p style="text-align: justify;"><span style="color: #ff0000;"><strong>Objectives ‘A’ </strong></span></p>
<ol>
<li><strong>Ashuddh garbha sambhava samagri </strong>&#8211; impure factors of fertilization &amp; conception-rutu (ovulation &amp; menstruation), kshetra (female reproductive system), ambu (body fluids), beeja (ovum, sperm &amp; zygote), <strong>in order to achieve shuddha garbha sambhava samagri (pure factors of fertilization &amp; conception).</strong></li>
<li><strong> </strong><strong>Arthava dushti </strong>irregular menstruation, sometimes amenorhoea and other time meno-metrorrhgia/oligomenorrhoea, dysmenorrhoea, <strong>in order to achieve shuddha arthava (purity &amp; alignment in menstrual cycle &amp; endocrinal co-ordination).</strong></li>
<li><strong>Shalyas</strong> -Physical, Mental, Emotional &amp; social Stressful factors, <strong>in order to attain prassannatmendriya mana (satiation of body, mind &amp; spiritual).</strong></li>
</ol>
<p style="text-align: justify;"><span style="color: #ff0000;"><strong> </strong><strong>Objectives ‘B’ </strong></span></p>

<p style="text-align: justify;"><strong> </strong>After the<strong> virechana karma followed by kala basthi</strong>, she had her menstruation on 28/12/2018 (after 6 months of amenorrhoea). Menstruation was uneventful, that is she did not suffer with any pains and liquid part was more than the clots in menstrual bleeding. Color of menstrual bleeding was light blackish red. Her inner garments were not stained.  5 Days bleeding with less small clots was observed by the patient.  Her weight also increased by 2kg after the kala basthi (from 58kg to 60kg). By this time, pimples had reduced completely. She was happy as her face was clear &amp; chubby cheeks.</p>
<p style="text-align: justify;">After the sthanika chikitsa (local per-vaginal treatment) through vaginal route, cervical erosion healed and healthy cervix noted.</p>
<p style="text-align: justify;">The two follow up examination showed- <strong>P/V- uterus –AVNS, no white discharge was seen. P/S- Cx –healthy.</strong></p>
<p style="text-align: justify;"><strong>She missed her menstruation on 28<sup>th</sup>January 2019. On 22<sup>nd</sup> February 2019 urine test for pregnancy was positive.</strong></p>
<p style="text-align: justify;"><strong>It was confirmed as intra-uterine pregnancy with gestation sac &amp; yolk sac, in ultra-sonography of abdomen &amp; pelvis. </strong></p>
<p style="text-align: justify;">Repeat scan was done on 14/03/19 which showed <strong>CRL-1.95cm, yolk sac was seen, fetal heart pulsation seen 178beats/min.</strong> <strong>Intra-uterine pregnancy of 8wks 4days.</strong></p>
<p style="text-align: justify;"><span style="color: #ff0000;"><strong> </strong><strong>Discussion:</strong></span></p>
<p style="text-align: justify;">Discussion is a detailed treatment of a topic in speech or writing, in order to reach a decision or to exchange ideas. Herewith I have tried to analyse the ayurvedic samprapthi &amp; treatments for the infertility due to PCOS &amp; ectopic pregnancy, with the help of our proud Acharya’s principles.</p>
<p style="text-align: justify;">It has been explained in our samhitas, the purity of arthava (menstrual flow) is the primary unit for garbhotpatti (fertilization &amp; conception). Our acharyas have highlighted that the menstrual blood does not leave any stain on the cloth.<sup> (26)</sup> This is the sign of purity in menstrual blood &amp; indicates the forthcoming ovulation.</p>
<p style="text-align: justify;">It was noted that, after course of deepana, pachana,virechana karma &amp; kala basthi karma, menstruation was uneventful, that is she did not suffer with any pains and liquid part was more than the clots in menstrual bleeding. Color of menstrual bleeding was light blackish red. <strong>Her inner garments were not stained.</strong>  5 Days bleeding with less small clots was observed by the patient.  This indicates the purity of the arthava menstrual bleeding.</p>

<p style="text-align: justify;">In Charaka Samhita Shareera Sthana 4<sup>th</sup> chapter named Mahati garbhavakranthi, commentator of Charaka Samhitha, Acharya Chakrapani has explained that, garbha gradually enters garbhashaya. However, if there is vikara in garbha, by the time it reaches kukshi or garbhashaya, it may get destroyed (<strong>kukshau jayamana; kukshau vinasham prapnoti</strong>) &amp; brings in death <strong>(katsnyernavinashyan iti marana gachchan) <sup>(27)</sup>.</strong></p>
<p style="text-align: justify;">Our Acharyas have written in sutra form. While explaining the early growth of fertilized ovum, Acharya Charaka has used the term <strong>kheta bhuta </strong>which means <strong>shleshma sadrashya-</strong> <strong>slippery in nature<sup>(28)</sup>,</strong> which is quiet similar to <strong>Zona pellucida</strong>-outer layer of the zygote. This layer helps the zygote, to slide down with its slippery nature, from fallopian tube to the intra-uterine cavity of the uterus for the implantation. One of the etiological factors for the tubal pregnancy is pre-mature loss of the layer zona-pellucida, loss of kheta bhuta can be compared to <strong>sthanika shleshma ksheena</strong>, when the fertilized ovum is in the ampulla part of the fallopian tube, as the fertilization occurs in the ampulla. Thus the zygote gets fixed to the fallopian tube, leading to rupture of the tube &amp; destruction of the zygote if goes unnoticed by the medical faculty. Thus tubal pregnancy gets destroyed &amp; increases the mortality rate of the mother.</p>
<p style="text-align: justify;">Subserosa layer of the fallopian tube is composed of loose adventitious tissue, blood vessels, lymphatics, an outer longitudinal and inner circular smooth muscle coats. This layer is responsible for the peristaltic action of the fallopian tubes. In turn this peristaltic movement helps in the gliding movements of zygote towards the intra-uterine cavity of the uterus. These movements can be compared to <strong>apana-vata,</strong> which is present in shroni pradesh &amp; is responsible for all the movements in the shroni. There is <strong>apana vata vigunata</strong> (improper function of apana vata). This is exhibited in the form of, irregular menstruation, sometimes meno-metrorragia other time oligomenorrhoea, dysmenorrhoea.</p>
<p style="text-align: justify;">The inner layer of fallopian tube is a single layer of simple columnar epithelium. The columnar cells have microscopic hair-like filaments (cilia) predominantly throughout the tube, but are most numerous in the infundibulum and ampulla. Estrogen increases the production of cilia on these cells. Between the ciliated cells are peg cells, which contain apical granules and produce the tubular fluid. This fluid contains nutrients for spermatozoa, oocytes, and zygotes. This can be compared to shleshma, rasadhatu. Tubal fluid flows against the action of the cilia, toward the fimbrial end. The fertilized ovum, now a zygote, travels towards the uterus aided by activity of tubal cilia and activity of the tubal muscle. Again these movements are responsibility of apana-vata.</p>
<p style="text-align: justify;">The early embryo requires critical development in the fallopian tube. After about five days the new embryo enters the uterine cavity and on about the sixth day implants on the wall of the uterus, which is again co-ordinate activity of vata-kapha-pitta in shroni &amp; minute srotas in shroni.</p>
<p style="text-align: justify;">Fallopian tube obstruction is associated with infertility and ectopic pregnancy. Risk factors for ectopic pregnancy include: pelvic inflammatory disease, often due to chlamydia infection, tobacco smoking, prior tubal surgery, a history of infertility, and the use of assisted reproductive technology. Those who have previously had an ectopic pregnancy are at much higher risk of having another one. Most ectopic pregnancies (90%) occur in the fallopian tube, which are known as tubal pregnancies (29).</p>
<p style="text-align: justify;">Presence of multiple pimples, heavy perspiration in palms and soles, often suffering from cold, constipation, irregular menstruation (more often amenorrhoea), anxiety and depression indicates the reduced immunity, accumulation of free-radicals in the body, disturbances at mental, emotional, spiritual and social levels due to loss of zygote in twice ectopic tubal pregnancies and unsuccessful IVF.</p>
<p style="text-align: justify;">Our acharyas have highlighted the importance of <strong>shuddh garbha sambhava samagri</strong> (pure sperm, ovum, uterus and bod fluids) to obtain a healthy garbha. And shuddhata of shareera &amp; manas (pure body &amp; mind) is also equally important.</p>
<p style="text-align: justify;">Acharya Sushruta while explaining swastha laxana, has given equal importance to equilibrium at <strong>physical level</strong>-shareera (samadhosha, samagni, samadhatu, sama mala &amp; kriyas of dosha-agni-dhatu-mala), <strong>spiritual level</strong> -prasannata of atma (satiation or brahmananda), <strong>mental level</strong> -prasannata of mana (happiness in all situations), <strong>emotional level</strong> -prasannata of indriyas (liveliness jnyanendriyas &amp; karmendriyas).</p>
<p style="text-align: justify;">In jathaharini adhyaya, it has been mentioned that, to get a healthy progeny, the couple have to follow <strong>‘Dharma palana ( Dharanath dharmaha)’</strong>that is good conduct to maintain health at physical, mental, emotional, spiritual and social levels<strong>. </strong></p>
<p style="text-align: justify;">By considering the above mentioned etiological factors, the exclusive treatment schedule was planned to target health at physical, mental, emotional, spiritual and social level. The plan is shown in the following table <strong>5</strong>.</p>

<p style="text-align: justify;">      After the Deepana Pachana for 2months, virechana karma was done after the samyak snigdha laxana with trivruth leha &amp; gandharva hasthadi taila respectively on 9<sup>th</sup> &amp; 10<sup>th</sup> day.</p>
<p style="text-align: justify;">On 15<sup>th</sup> day follow up after the virechana karma, her pimples had subsided. Sweating in palms and soles had reduced completely. Sneezing, running nose, cold had been subsided completely. Up to 1month same health was maintained.</p>
<p style="text-align: justify;"><strong>In second course, kala basthi karma as follows,</strong></p>
<p style="text-align: justify;">Anuvasana basthi- was done with Phala ghrita 75ml- for 9 days. On 6<sup>th</sup> 12<sup>th</sup> day of kala basthi karma, Gandharva hasthadi taila 50 ml- for 2days was used for Anuvasana basthi karma.</p>
<p style="text-align: justify;">Niruha basthi- was done with Honey 50ml+ Saindhava lavana10gms+ Shatapushpa churna10gms+ Phala ghrita75ml+ Eranda mula churna kashaya-300ml-for <strong>5d</strong>ays.</p>
<p style="text-align: justify;">After the completion of kala basthi karma, lady had her menstruation (after 6months of amenorrhoea) on 28<sup>th</sup> December 2018.  Lady reported that, her menstruation was uneventful, without any pains. Consistency wise liquid part was more than the clots in menstrual bleeding. The colour of menstrual bleeding was light blackish red. 5 Days bleeding with less small clots was observed by the patient. 3pads/day changed by the lady. Arthava (menstrual bleeding) highlights about homeostasis in the body, as arthava is the upadhatu (sub-part) of rasa dhatu. Homeostasis was attained. It also indicates the forthcoming ovulation.</p>
<p style="text-align: justify;">Activeness, good healthy raport with family and friends, involvement in her daily chores and smiling face throughout the day in lady (observed by her husband, friend and in follow up check-up). This indicates the stress free body, mind, emotions, spiritual and social well being in lady. Music therapy, surya namaskara, chinmudra pranayama are the parts of rasayana chikitsa. They have succeeded in returning her regular dinacharya. Music and yoga are the part and parcel of our daily work. For manasika shalya harsha chikitsa is indicated.</p>
<p style="text-align: justify;">From 8<sup>th</sup> day of menstruation that is on 5<sup>th</sup> January 2019, <strong>marsha nasya karma</strong> was started with anu-taila. The nasya treatment was done up to 11<sup>th</sup> January 2019. Councelling and music therapy were carried out on 5<sup>th</sup> and 11<sup>th</sup> January 2019.</p>
<p style="text-align: justify;">She missed her menstruation in January 2019. On 22<sup>nd</sup> February 2019 urine test for pregnancy was done at home by patient herself, which was positive for pregnancy. It was confirmed as intra-uterine pregnancy with gestation sac &amp; yolk sac, in ultra-sonography of abdomen &amp; pelvis.</p>
<p style="text-align: justify;"><strong>Repeat scan was done on 14/03/19 which showed</strong> <strong>CRL-1.95cm, yolk sac was seen, fetal heart pulsation seen 178beats/min.</strong> <strong>Intra-uterine pregnancy of 8wks 4days.</strong></p>

<p style="text-align: justify;"> Informed consent regarding documentation and publication of the case study was obtained from the lady &amp; her husband. Her husband has written about this case in review column in SDMIAH.  <strong> </strong></p>
<p style="text-align: justify;"><span style="color: #ff0000;"><strong>Conclusion: </strong></span></p>
<p style="text-align: justify;">Quality of life was hampered in lady due to irregularity in consuming food, sleep pattern, stress in day today life.</p>
<p style="text-align: justify;">Emotional stress had taken over her completely. Hampered quality life had negative effect on shuddha garbha sambhava samagri, leading to ashuddha garbha sambhava samagri (impure factors of fertilization), irregular menstrual cycles &amp; irregular flow, scanty flow.</p>
<p style="text-align: justify;">She was unable to conceive child. She had history of bilateral ectopic tubal pregnancy &amp; losing one tube in salpingectomy and the other tube with hydrosalpinx.</p>
<p style="text-align: justify;">Ayuveda has played very impotant role through dinacharya, virechana karma, kala basthi and marsha nasya karma, music therapy, yoga and pranayama, in obtaining shuddha garbha sambhava samagri (pure factors of fertilization) in the form of delivering a healthy female child through elective caesarian section. The aim and objects which was planned before starting the treatment, has been reached successfully.</p>
<p style="text-align: justify;">So, the term has been used as <strong>‘tapas by the couple’</strong> to get a healthy child.</p>
<p style="text-align: justify;">So the saying <strong>‘Child by choice &amp; not by chance’ </strong>should be taught to our people.</p>

<p style="text-align: justify;"><span style="color: #ff0000;"><strong>What is Parkinsonism?</strong></span></p>
<p style="text-align: justify;">Parkinsonism is a clinical syndrome characterised by tremor at rest, rigidity, postural an gait abnormality due to distinctive pathology of brain, where there is degenerative changes in brain stem nuclei including dopaminergic substantia nigra, pars compacta with presence of lewy bodies in the remaining cells. <strong>This disease is commonly known to affect the older age group</strong> i.e., over the age of 65 yrs. Early onset features can occur before the age of 40 years. It is said to be more common in men than in women in most populations.</p>
<p style="text-align: justify;"><span style="color: #ff0000;"><strong>Causes for parkinsonism</strong></span></p>
<p style="text-align: justify;">The exact cause for the disease Parkinsonism is unknown. It is believed that combination of several factors are involved in the development of Parkinsonism such as <strong>aging, environmental toxins such as exposure to pesticides and genetic predisposition.</strong>  There is an high risk in people who are exposed to some pesticides and also among those who have had earlier head injuries.</p>
<p style="text-align: justify;"><strong><span style="color: #ff0000;">Pathology involved</span> </strong></p>
<p style="text-align: justify;">The actual pathology involved in Parkinsonism disease is degeneration of substantia nigra (Present in brain), loss of dopaminergic neurons; which in turn lead to deficiency of dopamine leading to manifestations such as abnormal movements, tremors, impaired balance and posture; which are said to be the characteristic feature of Parkinsonism.</p>
<p style="text-align: justify;"><span style="color: #ff0000;"><strong>Diagnosis</strong></span></p>
<p style="text-align: justify;">More often the diagnosis of Parkinsonism is based on the clinical observations, the presenting signs and symptoms and detailed history. Other diagnostic aids such as CT and MRI brain can be used to rule out other disease.</p>

<p style="text-align: justify;"><span style="color: #ff0000;"><strong>Clinical Presentation of Parkinsonism</strong></span></p>
<p style="text-align: justify;">The most obvious early symptoms are tremor, rigidity, slowness of movement, and difficulty with walking. <strong>Parkinson&#8217;s disease dementia becomes common in the advanced stages of the disease. Depression and anxiety are common.</strong> Other symptoms include sensory, sleep, and emotional problems.</p>
<p style="text-align: justify;"><strong><span style="color: #ff0000;">Treatment</span> </strong></p>
<p style="text-align: justify;">There is no cure for Parkinson&#8217;s disease. It’s just treating the actual symptoms with drugs useful for treating motor symptoms are levodopa, dopamine agonists and MAO-B inhibitors. Other rehabilitative therapy like exercise programs are recommended in people with Parkinson&#8217;s disease</p>
<p style="text-align: justify;"><strong><span style="color: #ff0000;">Understanding Parkinsonism in</span> <a href="https://healthvision.in/category/articles/ayurveda/" target="_blank" rel="noopener noreferrer">Ayurveda</a></strong></p>
<p style="text-align: justify;">In Ayurveda each and every disease is understood in terms of Doshas Vata, Pitta, Kapha and this Parkinsonism disease based on its clinical features can be compared with that of Kampa Vata where predominantly there is impairment of Vata Dosha in the body. Literally the term Kampa Vata means, a disease caused by Vata Dosha having Kampa (Tremors) as a characteristic feature. Bench mark of Ayurveda specifies Karapadatale Kampa (shake in hands and feet), Dehabhramana (Instability in Postur), Mati Kshaya (Dementia), Stambha (Stiffness), Nidrabhanga (Distressed sleep), Chesta Hani (Slow movement), Vinamana (Bent Posture), and Vakvikruti (Speech chaos) as the clinical structures of Kampa Vata which accurately resembles with that of Parkinsonism Disease.</p>
<p style="text-align: justify;"><span style="color: #ff0000;"><strong>Treatment in Ayurveda</strong></span></p>
<p style="text-align: justify;">As the disease is mainly caused due to <strong>Vata Dosha Snehana (Oleation) and Swedana (Sudation) line of treatment would be beneficial.</strong> The specific line of treatment mentioned for Kampa Vata is Abyanaga (Oil Massage), Swedana (Sudation), Virechana (Purgation), Anuvasana-Niruha Basti (Enema Therapy) and Siro Basti (Retension of oil on head). Based on the intensity of Dosha Vitiation one can plan permutation and combination of these therapy according to the patient presentation.</p>
<p style="text-align: justify;"><span style="color: #ff0000;"><strong>Success case story</strong></span></p>
<p style="text-align: justify;">A Parkinsonism diagnosed; non-diabetic and non-hypertensive, male patient of age 68yrs hailing from Bangalore visited our OPD with complaints of bilateral shaking of hands and legs, postural disability, dementia, short step gait and unable to perform his day to day activities without any support. Suffering with these complaints since from last 4yrs. History revealed; no members in the family suffered from similar complaints, no history of trauma.</p>
<p style="text-align: justify;">The course of disease onset was gradual starting with shaking of extremities, difficulty in movements especially walking, sometimes suffered from constipation and increased frequency of urine.</p>
<p style="text-align: justify;"><span style="color: #ff0000;"><strong>Treatment given</strong></span></p>
<ul style="text-align: justify;">
<li><strong>Sarvanga Churna Pinda Sweda</strong> (Powder hot poultices therapy) with Jadamayadi and Kottamchukkadi Churna for 7days.</li>
<li><strong>Sadhyo Virechana</strong> (Purgative therapy) – Gandharva Hastyadi Taila – 40ml + Milk – 40ml</li>
<li><strong>Sarvanga Abhyanga</strong> (Oil Massage) + Patra Pinda Sweda (hot poultices therapy with leaves having medicinal properties)</li>
<li><strong>Eranda Mooladi Qwatha Basti</strong> (Enema therapy)</li>
<li><strong>Internally</strong> – Kapikachu Churna – 1tsp twice a day; with ½ glass of warm milk for 1 month</li>
</ul>
<p style="text-align: justify;">As it is a degenerative neurological disease there is necessity of treatments to be followed for multiple sittings for better and promising results.  Hence forth on 2<sup>nd</sup> visit; patient was treated with</p>
<ul style="text-align: justify;">
<li><strong>Sarvanga Churna Pinda Sweda</strong> (Powder hot poultices therapy) with Jadamayadi and Kottamchukkadi Churna for 7days.</li>
<li><strong>Sadhyo Virechana (</strong>Purgative therapy) – Gandharva Hastyadi Taila – 40ml + Milk – 40 ml</li>
<li><strong>Sarvanga Abhyanga</strong> (Oil Massage) + Patra Pinda Sweda (hot poultices therapy with leaves having medicinal properties)</li>
<li><strong>Rajayapana Basti</strong></li>
<li><strong>Internally –</strong> Ashwagandha Churna 1tsp; twice daily with ½ glass of warm milk for 1 month.</li>
</ul>
<p style="text-align: justify;">The signs and symptoms were markedly reduced and patient felt better by about   60-70%. Based on the clinical assessment on the next follow up treatment course will be decided.</p>
<p style="text-align: justify;"><span style="color: #ff0000;"><strong>Probable mode of action </strong></span></p>
<p style="text-align: justify;">Abyanga and Swedana treatment would counter act on Vata Dosha thereby reducing the symptoms such as tremors, rigidity, improving the posture and daily routine. (Pakvashaya) Large intestine is the main seat of Vata. Vata is one which controls all the neurological activities, Basti is the treatment applied directly to the main seat of Vata. Dhatukshaya abnormalities of Vata can be treated through Basti Karma.</p>
<p style="text-align: justify;">Vata is the chief cause of Kampavata when Vayu is controlled by the action of Basti all other disturbed body elements fall into almighty and equilibrium. <strong>Basti helps to strengthen the joints, soft tissue and rejuvenate the health.</strong></p>
<p style="text-align: justify;">Last but not the least I would like to say Ayurveda in right hands can be preventive, curative and front line treatment for many such aliments than just an alternative therapy.</p>

Observing this the questions comes to every mind, as,</p>
<ul style="text-align: justify;">
<li>Can’t we live healthy &amp; happily?</li>
<li>Can’t we leave behind healthy society &amp; environment?</li>
<li>Can’t we reproduce quality progeny?</li>
</ul>
<p style="text-align: justify;"><strong>The answer is ‘Yes’ to all the three questions.  </strong></p>
<p style="text-align: justify;">Theoretically positive answer is easy to recite. Practical application is also easy. However, the difficulty lies in practise by the people. Doctor/Vaidya plays an important role in all aspects. Especially ayurvedic doctors have more responsibilities.</p>
<p style="text-align: justify;">Answer to all the above mentioned 3 questions is explained as follows:</p>
<p style="text-align: justify;">In order to live happily, <a href="https://healthvision.in/category/articles/ayurveda/" target="_blank" rel="noopener noreferrer">Ayurveda</a> has contributed dinacharya, rutucharya and achara rasayanas to reach the goal, which is explained in the following shloka sutra:</p>
<p style="text-align: justify;"><strong>Samadosha samagnischa samadhatu malakriyaha I</strong></p>
<p style="text-align: justify;"><strong>Prassannatmendriya manaha swastha ityabhidheeyate II</strong></p>
<p style="text-align: justify;"><strong>Loka desham idam deha desham I</strong></p>
<p style="text-align: justify;">The meaning &amp; applied aspect of this sutra is, human is an epitome of this vast nature that is universe. Human physical body primarily is made up of tridosha (vata, pitta, kapha), or agnis (jataragni, dhatwagni, panchabhutagni), dhatus (rasa, raktha, mamsa, medas, asthi, majja &amp; shukra) &amp; malas (pureesha, mutra &amp; sweda). In every nook &amp; corner of the body dosha dhatu, agni &amp; mala should be maintained anatomically and physiologically.</p>
<p style="text-align: justify;"><strong> </strong><strong>Humans need to maintain peace &amp; satiation deep from the inner core of visible and invisible body so as to keep oneself in tune with the universal peace &amp; satiation</strong>. <strong>This will create sukhayu.</strong></p>
<p style="text-align: justify;">As human is an epitome of this universe, they need to be in par excellence with nature along with its other living &amp; non living things. Going in this track in parallel to everything in nature will create hithayu. This is the answer for the first &amp; second question. For the third question answer is explained in following paragraph:</p>
<p style="text-align: justify;">Through Ayurveda it is possible to have, quality reproduction.</p>
<p style="text-align: justify;">In Ayurveda it has been mentioned by our acharyas that in order to have a healthy progeny, four essential substances are required, called as garbha sambhava samagri. They are as follows:</p>
<ul style="text-align: justify;">
<li>Shuddha arthava</li>
<li>Shuddha shukra</li>
<li>Shuddha kshetra</li>
<li>Shuddha ambu</li>
</ul>
<p style="text-align: justify;">To attain this <strong>shuddhata,</strong> couple need to undergo certain <a href="https://healthvision.in/category/articles/ayurveda/" target="_blank" rel="noopener noreferrer">ayurvedic</a> treatment procedures in hospital. Along with this Couple need to maintain health at 5 levels. They are,</p>
<p style="text-align: justify;">Level one – Physical</p>
<p style="text-align: justify;">Level two – Mental</p>
<p style="text-align: justify;">Level three – Emotional</p>
<p style="text-align: justify;">Level four – Spiritual</p>
<p style="text-align: justify;">Level five – Social</p>

<p style="text-align: justify;"><span style="color: #000000;">Coriander is an aromatic herbaceous plant grown for its fruit and leaves in kitchen gardens as well as a commercial crop. The seeds (fruit) of this crop are used as condiment and as a major constituent of curry powder. The seeds and leaves contain essential oil rich in &#8220;linalool&#8217; which accounts for the aromatic character of the plant. The leaves are used as salad and are rich source of vitamins C and A. Used in the ailments like dyspepsia, flatulence and piles. </span></p>
<p style="text-align: justify;"><span style="color: #000000;">The oil is in good demand in other countries for flavouring liquors like gin and beverages and a variety of food products. </span><span style="color: #000000;">Coriander can be cultivated on almost all types ot soils provided sufficient organic manure is applied under irrigated conditions. Unirrigated crop may be cultivated on heavy soils which have better water retention capacity. Generally, it could be grown effectively on both light sandy and heavy loam soils.</span></p>

<p><span style="color: #000000;">2. Vomiting with indigestion- two spoons of the juice mixed with buttermilk is to be taken twice daily.</span></p>
<p><span style="color: #000000;">3. As it contains Iron, if taken with honey, the juice of the leaves improves the haemoglobin in anaemic patients.</span></p>

<p style="text-align: justify;"><span style="color: #000000;">Sometimes we consciously overlook the stress of our children in their path to excellence. It could be because we too are in the virtual rat race. When the parents are stressed out it reflects on the kids as well.</span></p>
<p style="text-align: justify;"><span style="color: #000000;">With the growing need for the future minds to ace in their subject of interest, more responsibilities lie on the shoulders of parents. For which it is utmost important that primarily the parents are keeping their inner peace. Through this article an attempt is made to share few health tips advises and lifestyle correction to parents to follow and instill in their children for a stress-free life.</span></p>

It is a very effective stress reliever; it improves concentration to deal with situations with a calm mind.</strong> It also helps to maintain body fitness and increases the immunity. <p style="text-align: justify;"><span style="color: #000000;">What most of us do not realize is that we have an effective inexpensive and safe remedy for anxiety and stress right in their hands. It is our Breath!!!</span></p>
<p style="text-align: justify;"><span style="color: #000000;">PRANAYAMA consists of two words, prana breath, ayama extension. It simply means extension of breath and thus the life through breath. Taking the attention to the breath can help free the mind of unnecessary clutter of thoughts that breed anxiety.</span></p>
<p style="text-align: justify;"><span style="color: #000000;">The other regimes mentioned in this article for de-stressing requires practice for a long term, but pranayama is the only one which gives instantaneous effect. There are different types of pranayama like the nadishuddhipranayama, sheetali, sheetakari and brahmari. Among these, brahmari benefits a lot for anxiety. Brahmari, when practised immediately alleviates anger, anxiety and helps for self-healing. The vibration of the humming sound creates a soothing effect on the mind and nervous system. You can get started right now.</span></p>
<p style="text-align: justify;"><span style="color: #ff0000;"><strong>To practice the Brahmari Pranayama just follow these simple steps:</strong></span></p>
<p style="text-align: justify;"><span style="color: #000000;">1. Sit in a comfortable meditation asana, preferably padmasana or siddha asana (sitting on the ground with folded legs) with hands resting on the knees in chin mudra(index finger tucked under thumb, other three fingers extended and palm facing upwards)</span></p>
<p style="text-align: justify;"><span style="color: #000000;">2. Close the eyes and relax the whole body.</span></p>
<p style="text-align: justify;"><span style="color: #000000;">3. The lips should remain gently closed with the teeth slightly separated throughout the practice. This allows the sound vibration to be heard and felt more distinctly.</span></p>
<p style="text-align: justify;"><span style="color: #000000;">4. Raise the arms sideways and bend the elbows, bringing the hands to the ears. Use the thumb finger to plug the ears or the flaps of the ears may be pressed without inserting the fingers.</span></p>
<p style="text-align: justify;"><span style="color: #000000;">5. Cover the eyes with remaining four fingers.</span></p>
<p style="text-align: justify;"><span style="color: #000000;">6. Bring awareness to the centre of the head, where ajna chakra is located, and keep the body still. Inhale through the nose.</span></p>
<p style="text-align: justify;"><span style="color: #000000;">7. Exhale slowly and in a controlled manner while making a deep, steady humming sound like a bee. The humming should be smooth, even and continuous for the duration of the exhalation. The sound should be soft and mellow, making the front of the skull reverberate. At the end of exhalation, the hands can be kept steady or returned to the knee and then raised again for the next round.</span></p>
<p style="text-align: justify;"><span style="color: #000000;">This is brahmari pranayama which leaves one totally mesmerised by its effect. Meditation is also an incredibly powerful tool for relaxing and slowing down the mind. But it is too much to ask the child to do meditation. Adults can follow it for relaxation.</span></p>
<p style="text-align: justify;"><span style="color: #ff0000;"><strong>Abyanga</strong></span></p>
<p style="text-align: justify;"><span style="color: #000000;">Application of oil and self-massage is the best way to remove the tension. It relaxes the mind and body, relieves stress meanwhile nourishing the skin and hair. Abyanga should be resorted to daily. It wards off old age, exertion, bestows good vision and promotes good sleep. Parents or grand-parents can help younger children with abyanga. Older kids and teenagers can be trained how to self-massage before bathing.</span></p>
<p style="text-align: justify;"><span style="color: #ff0000;"><strong>Snana</strong></span></p>
<p style="text-align: justify;"><span style="color: #000000;">Taking bath gives an instant zeal. It removes the tiredness from body and mind. It is rejuvenating and should be indulged every day. Whenever stressed, a bath again relaxes the nervous system, release tension and calms the mind.</span></p>
<p style="text-align: justify;"><span style="color: #ff0000;"><strong>Padabyanga</strong></span></p>
<p style="text-align: justify;"><span style="color: #000000;">Padabyanga is massaging the feet and legs with medicated Sneha (oil or ghee) focusing on the reflex points. Massage to the foot calms the mind, relaxes and revitalises the body. It removes fatigue, promotes sleep and one of the best remedy for stress. Massaging the foot before bed gives the desired effects. Padabyanga can be done by self in older kids once trained, in young children parents can help in the massage.</span></p>

<p style="text-align: justify;"><span style="color: #000000;">So common among women, yet the cause is unknown to us. And we are still creating theories, doing experiments in laboratories and fighting with this unknown bull. It just to women with no cause what so ever, and then your life change for forever. Form one doctor to another, from one medicine to another, the struggle continues.  You got me right; I am talking about the Polycystic Ovarian Syndrome (PCOS) &#8211; the puzzle of the century.  For some women it is like a lifelong curse where they experience all the worst symptoms. Managing problems such as Irregular periods, or diabetes, or obesity, hair fall, acne and hirsutism (unwanted body hairs) is itself difficult and takes a whole life to manage them. Think about a person who has got it all at once. Many PCOS women say to me, ‘I do not know how many births I need to take to overcome such difficult issues’. Well, in this article I am going to talk about hirsutism in PCOS and how Ayurvedic medicines can help you.</span></p>
<p style="text-align: justify;"><span style="color: #000000;">Neither all women with unwanted hairs have PCOS, nor do all the PCOS women suffer with these unwanted hairs on their body. Studies says, <strong>almost 80% of women with high levels of androgens (male hormones) have PCOS.</strong> By the name PCOS women generally believe there is serious problem only in their ovaries. They don’t see the doctor unless the symptoms of PCOS start troubling them especially if it is something to do with their external beauty. Irregular cycles do not bother them as much as this particular symptom take over their self-esteem, make them depressed and anxious. And that troublesome symptom is ‘Hirsutism- unwanted excess growth of hairs over face, chest and tummy’. Hirsutism is medically defined as the presence of terminal coarse hairs in females in a male-like distribution due to excess androgen activity.</span></p>
<p style="text-align: justify;"><span style="color: #000000;">Let me tell you a story of one of my patients named Neela (imaginary name). Neela was in in her early 20’s when she visited me couple of months back. She was covering her head with a pink color silk scarf. I asked her, what brought you here to see me? Neela removed her scarf and showed me her chin and jaw line. ‘Look at this doctor, I have done it all to stop it but nothing is helping me to overcome these excess facial hairs. I feel terrible and ashamed’. I enquired in bit detail about Neela’s issue. She was not obese but definitely she had lot of fat around her tummy. I asked if she has been diagnosed with any medical issues, such as diabetes or PCOS?</span></p>
<p style="text-align: justify;"><span style="color: #000000;">She promptly said, ‘yes, I was diagnosed with PCOD (age of 15) right after one year I had my first cycle. My cycle was irregular with very less flow. I was told that it takes time for a cycle to settle due to hormonal changes in my body. I did not care about that as my cycle became regular slowly however, flow was anyway less only. But one day, I was shocked to see 2-3 long thick hairs on my chin. Also, I started visiting parlor for threading and waxing frequently than ever. I was sure something was not right. I went to a nearby doctor with the complaint of thick and dark hairs on my upper lips, side of the cheeks and over chin area, she said anxiously’.   These were only visible hairs. She also had such masculine hairs over her other body parts, which were easy to hide.</span></p>
<p style="text-align: justify;"><span style="color: #000000;">I was given some pills to regularize my hormones and asked to do some diet and exercises, although I was not fat at all. I took those hormonal pills (OCP) for one year. I was hoping to recover as I could see some flow during cycles and that heavy hair growth were thinning down. It was time to celebrate. I thought I am out of this curse and I will get my beautiful face back. But, good things never last long.   As soon as I discontinued the pills, my skin broke down. I got much thicker and heavier hairs over my chin and this time they spreaded although my jaw line. My mother took me to another doctor who diagnosed me with PCOS. I did not know what it initially. My mother thought I have got some serious problem. I was confused. Google came to help me understand it better. I remember, I had spent almost 1 week or two just reading about PCOS on internet. I was continuously asking myself, how on earth how got this problem. No one in my family had this. I accepted it as my bad karma and took medications given by the new doctor for 6 months. I was even bullied in my college for having such unnatural hair growth. I stopped all the medication and finally learned that there is no end to this. I concentrated on my carrier and forgot about PCOS. I even took some anti-androgen for 6 months with no better results.  I will never forget those days. Leela stooped taking after saying this. She appeared frustrated and sad.</span></p>
<p style="text-align: justify;"><span style="color: #000000;"><img decoding="async" class="wp-image-1298  aligncenter" src="https://healthvision.in/wp-content/uploads/2018/06/1-e1528208972777.jpg" alt="patient with PCOS." width="435" height="331" />Looked like, Leela had finally accepted her defeat with PCOS. She just wanted some cosmetic relief with her facial hairs. I talked her about Ayurvedic concepts of looking at her health issues. I informed that Ayurveda does not look at an issue as a separate entity rather it takes a holistic approach considering you have got this issue and not just your body parts. You need healing and not just your face or ovaries. I asked, do you trust that Ayurvedic medicines would be helpful to you? She nodded quite positively. She said, I don’t have any other option. At least I will not have any side effects with these’.  I asked her to come back tomorrow on empty stomach to check her nadi and then I can provide her remedies for her health issues. She was wondering why I need to check her nadi when she just wants to get rid of facial hairs and not PCOS. She asked doctor please give me some ointments or powders so I don t see theses hairs ever again. The quick solution is what everyone wants. Who wants to suffer from an illness for so long? Fair enough but there is a way to heal and not just to fix, I firmly advised her.</span></p>
<p style="text-align: justify;"><span style="color: #000000;">She agreed and came to me next day morning 9 am. I thought she would not come back considering I may take long time to heal her. Leela said, the only thing that bothers me more than anything is these facial hairs. I go to beauty parlor twice-thrice in a month to get a wax done. It is very shame full some times when my friends points towards to my chin and get curious  to know if there is any wrong with me. I have also taken laser treatment but that did not help me to get my feminine face back, moreover I cannot afford laser, electrolysis and expensive hormones.  Because of this facial hairs my relations friends and family is challenged due to myself low self-esteem. More over who will marry a girl with a beard and mustache. Please help me with Ayurveda.  I have read all your articles and have full trust in you, she said with confident.</span></p>
<p style="text-align: justify;"><span style="color: #000000;">I do not know what was changed in Leela in one day. I checked her Nadi and examined her as per Ayurvedic parameters. History taking is always important which describe both how the patients is and how the disease is. For Leela, it was severe Kapha and Vata imbalance. Her Pitta was too low, Agni was weak and lot of Ama (toxins) was collected in her gut. Leela was opened for the treatment. I explained her that she needs Panchakaram. This will help her to clean her body with abnormal toxins (AMA), support her Agni though Deepana and Pachana and then will start internal medication. After 45 days of cleansing (Panchakarma) , I gave her medicines which contained herbs such as harida, tulasi, ashwagandha, shatavari, yastimadhu, sharapunkha, Jatamansi, tankana, Cinamon, fenugreek and tulasi , Yasti , Shatavari , Gokshura, Varuna and guggulu. I also prepared a powder and oil for her to apply externally on her hair follicles of chin and jaw line after she waxes them which helped to reduce the thickness and growth of hairs. She took the treatment for 1 year with satisfied results, slowly I discontinued few herbs and she was able to manage her symptoms effectively than before with more positive outlook towards PCOS.</span></p>
<p style="text-align: justify;"><span style="color: #000000;">This is not the only story I have. Many young girls are now suffering with this issue. At times, I have seen them complaining about such abnormal facial hairs since they get mature. Well, that is serious agony. Also think about a young hard working professional who just got a high profile job in a reputed company is struggling to keep her face ‘hair free’. And it seems her life will almost end if she does not wax or thread those long hairs over her chin. No one to blame!</span></p>
<p style="text-align: justify;"><span style="color: #000000;">You may already know is that this is all because of high levels of Androgens (Male hormone) in your body or so called ‘too much active’ testosterone. Androgens are known to be responsible for promoting hair cycles among men and women. Males generally have high levels of androgen and thus have larger hair follicles- mustache and beards etc.  Every woman needs male hormone in their body to trigger their female hormone – estrogen to work efficiently. But if this male hormone is excess then that is where women start feeling symptoms of being  ‘masculine’ – belly fat, hard skin, hair fall, growth of men like hairs over body and evens some develop deep voice like men.</span></p>
<p style="text-align: justify;"><span style="color: #000000;"><strong><img decoding="async" class="wp-image-1297  aligncenter" src="https://healthvision.in/wp-content/uploads/2018/06/3-e1528208873784.jpg" alt="flow chart of hormonal changes" width="435" height="312" />For most <a href="https://healthvision.in/category/articles/women-health/" target="_blank" rel="noopener noreferrer">women</a> with PCOS this male hormone levels are higher than normal.</strong>  Doctor may recommend a simple blood test and may find that your free testosterone levels are high in your blood. However, hirsutism can be present with normal ovulatory function and normal androgens level. High levels of free testosterone in your body prevent ovulation and that’s why you may not be getting pregnant if you are trying.  If you are diabetic then you have got to work on that too as insulin too worsen the male hormone levels.</span></p>

Acharya Kashyapa has given very precise description of Pushpaghni Jatiharini, which is almost having the symptoms of PCOS as told by modern science. Well, I am not comparing the two distinct sciences but trying to find a bridge where you can come to an agreement with me. The reference goes like this-</span></p>
<p style="text-align: justify;"><span style="color: #000000;"><strong><em>“Vritha pushpam tu yo nri yathakal prapashyati. Sthulalomashganda vpushpaghni sa api revati” </em></strong><strong><em>(Ka S.K.6/32-33)</em></strong></span></p>
<p style="text-align: justify;"><span style="color: #000000;">It is curable. Woman menstruate in time but it is useless (<em>vyathpushpa </em>i.e anovulatory cycle). She has corpulent and hairy cheeks –hirsutism; may be due to hyperandrogenism.</span></p>
<p style="text-align: justify;"><span style="color: #000000;">So what you need to overcome these unwanted hairs is to look after your hormones. Balance your doshas and Agni.  The healthy diet will help you much extend. Use the anti-androgen that causes unwanted body hairs in PCOS. If you are obese then lose some weight. Check your blood sugar levels, if you are insulin resistance. Eat healthy food and excise’s to improve your insulin and glucose control. This will help your ovaries to reduce the male hormones’ secretion.</span></p>
<p style="text-align: justify;"><span style="color: #000000;">Let me talk more about Almonds here, research says that they tend to reduce the potency of male hormones-testosterone. Again, many women with PCOS have high levels of male hormones in their body. So, <a href="https://amzn.to/3mQHcTq" target="_blank" rel="noopener noreferrer">eat 5-6 almonds</a> soaked the previous night and munch on them. For external application- apply <a href="https://amzn.to/3crdFee" target="_blank" rel="noopener noreferrer">haridra churna</a> and chickpeas flour together on the hair follicles.</span></p>
<p style="text-align: justify;"><span style="color: #000000;"><strong>What your food should have- </strong>The best thing to start your day is to have Vata-Kapha balancing Ayurvedic herbal kashaya (tea).</span></p>
<ul style="text-align: justify;">
<li><span style="color: #000000;"><strong><em>Your breakfast </em></strong>should have Kapha balancing foods, such as Barley or Horse gram soup, Whole wheat Roti, Ragi, and 1 handful of nuts/seeds. Avoid- fermented food items such as dosa, idli, vada and white rice early mornings.</span></li>
<li><span style="color: #000000;"><strong><em>Kapha balancing snack- </em></strong>buttermilk is best with ginger, also can have 1 apple or papaya.</span></li>
<li><span style="color: #000000;"><strong><em>Lunch- </em></strong>should have small portion of brow\n rice, lots of veggies or meat or sea food and salads.</span></li>
<li><span style="color: #000000;"><strong><em>Vata balancing snack</em></strong>&#8211; in the evening it is best to the herbal tea again or handful of nuts/seeds or just 1 glass of milk.</span></li>
<li><span style="color: #000000;"><strong><em>Dinner- </em></strong>should have lot of different vegetables and roti if required. Also can have cooked brown rice, quinoa, porridge, low sugar baked beans, red kidney beans, lentils or chick peas. Avoid having rice and heavy meals in the night.</span></li>
</ul>
<p style="text-align: justify;"><span style="color: #000000;">Remember beauty doesn’t live in the physical appearance, it lies within you. It is more important to understand how do one feel rather then we look. </span></p>

However, in miniature form universe is present in human body. Therefore, we need to be in good co-ordination with the universe or nature. As our acharya Sushruta has genueinly stated that swastha means balance of dosh, dhatu, mala, agni (physically &amp; biologically) and prasannata ( liveliness/enthusism/satiation) of soul (Atma) mind (manas). In this present era, as the universe is being polluted, so also humans are polluted mentally, physically &amp; biologically, leading to unhealthy conditions or situations, due to their food habits or living style. Due to this pollution and tensed environment, her thesaurus is increased. In playing &amp; maintaining the different role, her energy is decreasing gradually &amp; she feels defeated in her surroundings. However, her tasks are increasing day by day, along with this she has to maintain her pretty smiling face.</p>
