Also known as dermatitis, eczema is among the commonest forms of skin disease, and it often responds well to Homoeopathic treatment. It affects up to 20 per cent of school children and seven to eight percent of adults. Children tend to grow out of it and the majority improve greatly by their mid-teens. It is an inflammatory condition in which patches of skin become red, inflamed and itchy. The affected areas may also be covered in small, fluid-filled blisters. • The commonest form is <strong>atopic dermatit</strong>is, an inborn condition which usually starts before the age of two, and may continue to flare up throughout adolescence and adulthood. &#8220;Atopy&#8221; means an inherited tendency to allergies, including eczema, asthma and hay fever. It runs in families and is getting commoner for reasons that are not entirely clear.<br />
• <strong>Contact dermatitis</strong> is caused by contact with a substance to which an individual is sensitive, such as nickel, rubber or various plants. It can occur at any age. Nickel is one of the commonest skin sensitizers &#8211; it can usually be spotted by the areas it affects: earrings and jean buttons often contain nickel.<br />
• <strong>Seborrhoeic dermatitis</strong> occurs in adult and infantile forms. In babies it is often known as cradle cap because it affects the scalp, although it may also affect the nappy area. For adults it usually affects the face and scalp.<br />
• Finally, <strong>varicose eczema</strong> occurs mostly in older people, as the name suggests it is often linked to varicose veins: the blood stagnates in the lower legs resulting in a poor supply of oxygen and nutrients. The skin becomes dark, itchy and inflamed. If left untreated the skin may break down, forming an ulcer.</p>
<p style="text-align: justify;"><strong><span style="color: #0000ff;">Psoriasis</span></strong><br />
This too comes in several varieties, but all involve reddened areas of skin covered in large, adherent silvery scales. Psoriasis tends to run in families; it most commonly comes on in late teenage or early adult life. It can be triggered by some medicines, Including antidepressant, antihypertensive &amp; antimalarial drugs, &amp; infections with streptococcus bacteria.</p>

It is a condition in which the over-active oil glands and dead cells clog the pores resulting into pimples. It mostly occurs in teen age. They mostly involve the face, neck, back and shoulders. If left untreated it may cause blemishes and scarring.</p>

The common raised bumps on the skin known as warts are actually caused by the human papillomavirus (HPV). Warts are contagious, and can appear on any part of the body. Common warts typically grow on the hands, feet, and joints, though they can appear anywhere. Usually have a hereditary origin.</p>
<p style="text-align: justify;"><span style="color: #0000ff;"><strong>Fungal nail infection</strong></span><br />
A condition in which fungus lives near, under, and around the nails, usually in the feet. The fungal build-up causes the nail&#8217;s edges to crumble away, producing white-yellowish scaling and flaking on the surface of the nails.</p>

Lupus is a complex disorder that varies from person to person. The disease attacks the immune system, causing inflammation and pain. While lupus can affect any part of the body, symptoms on the skin include red patches or ring shapes on the skin, sunburn-like rashes on the nose and cheeks, or circular rashes that don&#8217;t itch or hurt. These may be accompanied by other symptoms, such as headaches, fever, fatigue, and swollen, stiff, or painful joints.</p>

Shingles, also known as herpes zoster, is a skin disease caused by the return of a chickenpox infection from latently infected nerve cells in the spinal cord or brain. It is soon followed within one or two days by a red, blistering unilateral (one-sided) rash distributed to the skin supplied by a sensory nerve (a dermatome). <p style="text-align: justify;">Vitiligo is a long-term condition where pale white patches develop on the skin. Melanin is the pigment present in the skin. The lack of this pigment causes vitiligo. Although <strong>vitiligo can affect any area of the skin the most common occurrences are prevalent on the face, neck and hands and skin creases.</strong> It can sometimes start in the lips, tips of fingers and genital areas also. Vitiligo affects people of all skin types, but it may be more noticeable in people with brown or black skin.</p>
<p style="text-align: justify;">This disorder is not lethal or infectious and though considered an autoimmune disorder can be rarely associated with disorders in other endocrine organs like <strong>thyroid or adrenal glands</strong>. However, the appearance of vitiligo can cause stress and low self-esteem. It&#8217;s challenging to foresee how this disease will develop. Sometimes the patches stop forming without treatment. In most cases, pigment loss spreads and finally involves most of the skin. In rare cases, the skin gets its colour back.</p>
<p style="text-align: justify;"><span style="color: #ff0000;"><strong>Signs and symptoms</strong></span></p>
<p style="text-align: justify;">The symptoms of vitiligo can appear at any phase of life. The signs include:</p>
<p>1. Loss of skin colour in patches, usually seen first on your face, hands, arms, legs, and feet</p>
<p>2. Early paling of your body hair, including hair on the lashes brows and face if the areas are involved.</p>
<p>3. Mostly it is asymptomatic and there are no symptoms like itching or pain.</p>
<p>4. Thyroid and Adrenal gland problems.</p>

<p style="text-align: justify;"><span style="color: #ff0000;"><strong>Detection of vitiligo</strong></span></p>
<p style="text-align: justify;">A dermatologist will be able to detect vitiligo post skin examination. The doctor will look for symptoms associated with other skin conditions, such as other hypopigmentary disorders like common eczemas or healing lesions of psoriasis etc in order to rule them out. The doctor will probably look at every area of the skin in order to determine which type of vitiligo you have based on where the patches appear. On dark skin, detection of patches is easy. But for lighter-skinned people, the doctor might use a tool called a <strong>Wood lamp (which emits ultraviolet light)</strong> as there is less distinction between the affected skin and unaffected skin. Skin with vitiligo will look different under this type of light. The tool can also be used to differentiate between loss of pigment and reduction in colour.</p>
<p style="text-align: justify;">Lightened patches appearance and medical history of the patient is adequate for the diagnosis of vitiligo. However, doctors may recommend additional tests for further confirmation. For example, a skin biopsy will show whether melanocytes are present. <strong>Non-existence of melanocytes can confirm a vitiligo diagnosis.</strong></p>
<p style="text-align: justify;">The skin biopsy can also indicate whether the patient is dealing with a skin cancer called hypo pigmented cutaneous T-cell lymphoma. Doctors often recommend a blood test to check if the vitiligo is related to an underlying autoimmune disease, such as anaemia or type 1 diabetes. Because of the association with other autoimmune diseases and endocrinopathies, further testing may be necessary in patients with indicative signs or symptoms to rule out an underlying condition. <strong>Vitiligo may be associated with thyroid disease, diabetes mellitus, pernicious anaemia, addison disease, and alopecia areata.</strong></p>
<p style="text-align: justify;">There is no cure as such for vitiligo but proper treatment can halt or slow the discolouring process and return some colour to the skin. The aim of the procedures is to create an even skin tone by either restoring colour (repigmentation) or eliminating the remaining colour (depigmentation). Camouflage therapy, repigmentation therapy, light therapy and surgery are some of the common treatments to cure vitiligo.</p>

<p style="text-align: justify;"><strong>4. Medications and dietary supplements-</strong> medicines which are used to treat cancer, arthritis, depression, heart issues, gout, and high blood pressure, can also cause hair loss.</p>
<p style="text-align: justify;"><strong>5. Nutritional deficiencies</strong> can result from poor nutrition or following a severely restricted crash or fad diet, which can lead to hair loss ranging from thinning to patches of baldness.</p>

Rubber bands, barrettes, and braids put a lot of strain on your hair, so try to avoid them.</p>
<p style="text-align: justify;"><strong>2. Stay hydrated &#8211;</strong> The hair shaft is made up of one-quarter water, so drink at least four to eight glasses of water every day to stay hydrated and promote healthy hair development.</p>

<p style="text-align: justify;">Even with proper care, the skin may still develop problems, such as wounds or pressure ulcers or bedsores or IAD (Incontinence Associated Dermatitis). Often the changes to the skin can&#8217;t be prevented or healed however, there are ways to manage wounds and bedsores to keep a patient comfortable. Bedsores that develop at this stage get worse quickly and often don&#8217;t heal.</p>
<p style="text-align: justify;"><strong>The high incidence and prevalence of bedsores confirm the importance of palliative skin care</strong> focusing on prevention and management strategies. They are driven by patient and family goals integrated with three components of wound management:</p>
<p>1. The management or palliation of the underlying cause of the wound</p>
<p>2. Management of wound-related symptoms</p>
<p>3. Management of the wounds itself</p>
<p style="text-align: justify;">In addition, meticulous skin care for patients of all ages with debilitating long-term conditions is crucial to prevent unnecessary skin breakdown. The key clinical interventions include the application of wound dressings and skin care products.</p>
<p><span style="color: #ff0000;"><strong>1. Palliative Care Principles of bedsore, wound care and prevention </strong></span></p>
<p style="text-align: justify;">The goals of palliative wound care are stabilization of existing wounds, prevention of new wounds (if possible), and symptom management of complications to improve patient comfort, well-being, and quality of life.  The general recommendations for four major areas of wound prevention: repositioning, support surfaces, skincare and nutrition.</p>

Repositioning for all persons at risk of developing bedsore, the frequency and techniques of which should be individualized to achieve maximal comfort, dignity, and functionality. Any kind of movement while repositioning or helping patient with daily chores may increase friction and shear forces on the skin over the sacrum and bony prominent areas. Application of new age silicone barrier gels in such pressure prone areas may help prevention of friction and shear while such movement.</p>
<p><strong><span style="color: #ff0000;">3. Support Surfaces</span>  </strong></p>
<p style="text-align: justify;">Selection of a surface to reduce pressure over bony prominences should take into account the patient’s level of mobility in bed, his or her comfort, and other environmental conditions.  Support surfaces include mattresses; overlays, which can be filled with air, water, gel, foam, or a combination of these materials which are designed to provide pressure relief on the body.</p>

<p style="text-align: justify;">Malnutrition, which is highly common in the palliative care setting, is known to increase the likelihood of bedsore formation and can be a reason why wounds fail to heal or become worse. Nutritional assessment is indicated in palliative care of elderly adults. Additionally, certain medications or symptoms of chronic conditions can affect the absorption of nutrients. As a result, the need to encourage the patient to eat and drink frequently can outweigh other dietary restrictions. One approach to increasing nutritional intake is to offer the patient their favourite food or beverage (within reason).</p>
<p style="text-align: justify;">The importance of water in nutritional status should not be overlooked, as it has been demonstrated that dehydration may increase the risk of bedsore formation and delay healing.</p>
<p><span style="color: #ff0000;"><strong>5. Skin care in Palliative Setting</strong></span></p>
<p style="text-align: justify;">Excess dryness or moisture can make the skin more susceptible to bedsore or wounds. Prevention might involve assessing the state of the skin and applying barrier creams or gel or medicated lotions in case of wounds to the area. At this stage of life, prevention is best form of cure and regular use of barrier creams or gels is recommended to maintain skin integrity. Barrier creams or gels are one of the most effective prevention strategies to manage bedsores. They act as a lubricating film on the skin. Barrier creams help maintain the skin’s physical barrier and stop trans-epidermal water loss. These have protective effects against skin irritants and work by inhibiting direct contact of skin to harmful body fluids such as urine or faecal matter that may cause incontinence associated dermatitis or bedsores.</p>
<p style="text-align: justify;">Patients who experience excess perspiration may need frequent clothing changes or bed linen changes along with application of barrier creams to prevent moisture build-up that can lead to sores.</p>
<p style="text-align: justify;"><span style="color: #ff0000;"><strong>Conclusion</strong></span></p>
<p style="text-align: justify;"><strong>Caring for the patient with serious illness or patients at end stage of life presents many challenges</strong>, not least the maintenance of skin integrity. Occasionally, despite receiving all of the necessary interventions, patients will still develop damage to their skin. These including but not limited to bedsores are now being recognised as inevitable skin changes at life’s end, however, it is still vital that all preventative measures are taken to manage such patients and try to restore quality of their life including palliation of symptoms, provision of optimal care, and using appropriate bedsore prevention techniques.</p>

As with all activities that involve fun, there are a few practices that are not totally safe and healthy.</p>

To prep the skin, one can use a thick hydrating cream and ensure that the skin is well-hydrated, this will help protect the skin from the chemicals and partially prevent the chemical from penetrating the skin. On the body apply a light oil like almond oil. It contains vitamin E and provides nourishment to the skin and acts as a protective layer between the skin and the colours.</p>
<p style="text-align: justify;">And top it off with a waterproof sunscreen which will help prevent skin damage from heat, the sun and colours. Physical sunscreen containing zinc oxide is safe and effective. <strong>Stay Hydrated.</strong> As you are mostly outdoors and under the sun, keeping yourself hydrated is much needed. <strong>Avoid wearing too much makeup as the chemicals that are present in your cosmetics can react with the colours further causing a skin reaction.</strong></p>

Post Holi skin care is essential to nourish the skin, prevent damages and get back that natural glow. Gently massage the skin with light oil to take off the colour and wash off with a mild cleanser. A quick luke warm water bath to wash off the colour easily. <strong>Avoid scrubbing as it may dry out the skin and make it itchy and uneven.</strong>  Seal the skin with a hydrating moisturiser soon after bath.</p>
